Clinical Application Of Transurethral Plasmakinetic Resectoscope And Retroperitoneal Laparoscopy In Radial Resection Of The Renal Pelvic Carcinoma

Objective:To explore the clinical application and effects of transurethral plasmakinetic resectoscope and retroperitoneal laparoscopy in radial resection of the renal pelvic carcinoma, so as to provide reasonable scheme for treatment and rehabilitation of patients with renal pelvic carcinoma. Methods:A retrospective analysis was carried out on 48 patients with renal pelvic carcinoma who were admitted in urology department of Hospital affiliated to Qinghai University between March 2008 and March 2014. The patients were divided into the minimally invasive surgery group(n=20) and open surgery group(n=28). The minimally invasive surgery group was given radial resection of the renal pelvic carcinoma with transurethral plasmakinetic resectoscope and retroperitoneal laparoscopy while the open surgery group was given traditional radial resection of the renal pelvic carcinoma. The two groups of patients were compared for intraoperative blood loss, hospital stay, incidences of complications, recurrence rate, tumor deposit rate and postoperative quality of life. SPSS17.0 was used for data analysis.Results:Intraoperative blood loss and hospital stay were 85.6±16.5 m L and 12.5±3.2 days in the minimally invasive surgery group, which were less than those of the open surgery group(122.7±18.6 m L and 18.5±4.4 days, P<0.05). The incidence of complications was 10% in the minimally invasive surgery group, which was less than that of the open surgery group(35.71%, P<0.05), and the difference was of statistical significance(P<0.05). And the differences of recurrence rate and tumor deposit rate were not significant(P>0.05). Total scores of life quality were increased in both of the two groups after treatment. And there were statistically significant differences between pre-treatment and post-treatment values(P<0.05). There were no obvious differences between the two groups in total scores of life quality before treatment(P>0.05), while there were obvious differences after treatment(P<0.05). And the total scores of life quality in the minimally invasive surgery group had a greater ascending range. Conclusions :1. Combination therapy of transurethral plasmakinetic resectoscope for resection of ureteral inner segment and retroperitoneal laparoscopy for resection of ureter has less blood loss, short hospitalization time, and small trauma, fewer incidences of complications, high operation safety, and fast recovery than traditional open radial resection for the renal pelvic carcinoma, suggesting the combination therapy can effectively reduce the incidences of complications. Recurrence of bladder tumor and retroperitoneal tumor deposit rate are equal, suggesting the combination therapy does not increase the recurrence of bladder tumor and retroperitoneal tumor deposit rate. It can significantly improve the patients’ quality of life. 2. The operation mode conforms to the principle of oncology. It is minimally invasive, effective, and safe, which is worthy of clinical popularization and application.
