Objective To assess the clinical value and advantages of HPD in the treatment of BPH, by comparing its clinical effects with PKEP.Through the research, analysis and transurethral resection compared, transurethral HPD in the operation time, bleeding volume during operation, preoperative, intraoperative, postoperative electrolyte changes, intraoperative flushing fluid volume, indwelling catheter time, hospitalization time, postoperative complications, and so on is superior to transurethral resection, and then provides the powerful help for clinical selection of appropriate treatment or operation mode.Method Several patients with BPH of Hebei NO.7 people’s Hospital between December,2013 and November,2014.Based on the different treatment methods, selected 30 cases of patients by HPD divided into experimental group, another 30 cases of patients by PKEP divided into the control group.The 60 patients had symptoms of dysuria, and lower urinary tract obstruction. The International Prostate Symptom Score(IPSS) score more than 20 points, the quality of life questionnaire(QOL) score more than 4 points,maximum urinary flow rate(Qmax) of less than m L, Through the B ultrasound or MR examination,prostate volume is greater than 60c;digital rectal examination of the above;non neurogenic bladder,unstable bladder,bladder stones and bladder tumor; PSA was less than 4ng/ml, MRI or biopsy to excludeprostate cancer;PSA was less than 4ng/ml or MRI,biopsy to exclude prostate cancer Comparative study of operation time and intraoperative blood loss,postoperative bladder washing time,time of indwelling urinary catheter,postoperative complications,hospitalization time,cost and other differences. test and t test were selected according to the data types. When P<0.05, it had statistical significance. All analyses were performed by the software SPSS 16.Result All operations were successfully done.1.General informations of HPD group and PKEP group compared were similar, body mass index and the severity of the disease and many other aspects(P>0.05), the difference was not statistically significant,indicating that the two groups were comparable. 2.In comparion of HPD group and PKEP group,the average duration of operation was 60.9±11.9(min) for HPD, and 61.3±10.7(min) for PKEP,There was no significant difference(P>0.05). 3.The average intraoperative blood loss was 33.4±16.8(ml) for HPD, and 67.8±29.7(ml) for PKEP; The average postoperative bladder irrigation time was 1.9±0.7(d) for HPD, and 3.5±1.7(d) for PKEP;The postoperatively average indwelling catheter time was 1.5±0.8(d) for HPD, and 4.7±2.2(d) for PKEP.The postoperatively average length of stay(LOS) was 3.9±1.3(d) for HPD, and 5.8±2.4(d) for PKEP.The intraoperative blood loss,postoperative bladder irrigation,duration of indwelling catheter,LOS of HPD group was significantly lower than that of PKEP, and the difference had statistical significance(P<0.05).4.In PKEP group: secondary bleeding 1 cases,bladder neck contracture 1 cases, urinary tract infection 1 cases, short-term uracratia 1 cases, occurred in the 30 cases of patients after operation, postoperative complications was 13.3%; HPD group of 30 cases after operation,urinary tract patients with infection 1 cases,short-term uracratia 1 cases, the complication rate was 6.7%.The postoperatively average incidence of complications for HPD was significantly lower than that of PKEP too, and the difference had statistical significance(P<0.05).Conclusion HPD was more simple, more safe, less trauma and more curative effect than PKEP in treating BPH. Because of it has less blood loss, shorter bladder washing time,shorter time of indwelling urinary catheter,shorter hospitalization time and less complications. It could be a better alternative surgical method and be desirable to be further popularized for treatment of BPH.
