Objective To study the correlation of prognosis and the S100 B levels of cerebrospinal fluid and serum in moderate and severe traumatic brain injury,and the correlation of the S100 B levels between CSF and serum at the same time point.Method According to the selection,exclusion criteria,select eligible TBI patients as the research patients.Select the patients of no brain injury,who had long bone fracture internal fixation of 3-6 months after operation,also need remove the medical steel plate,as the healthy control group.According to the admission GCS, the TBI patients were divided into the moderate group and the severe group.According to the GOS after injury,patients were divided into 1-2 score group, 3-4 score group and the 5 score group.Collect CSF and serum samples after admission at 1,3,5,7,14,21 day,then detect the level of S100 B by ELISA method.Results 1.The mortality of cerebrospinal fluid cohort(152 cases) was 10.5%,and the serum cohort(168 cases) was 11.3%.2.The S100 B levels of cerebrospinal fluid and serum decreased gradually with the time.Peak comcentrations occurred on 1d and 3d in both serum and cerebrospinal fluid.The S100 B levers of cerebrospinal fluid and serum at each time point were higher than that of healthy control group(P<0.05).In the cerebrospinal fluid,the S100 B levers in the first day were significant higher those that of the third day(P < 0.05).Compared with cerebrospinal fluid,the S100 B values declined in the serum was not as marked, but the concentrations in the first day was significant higher than those of the third day,and also significant higher than thoset of the fifth day(P < 0.05).3.CSF/S100 B correlation was stronger in a certain period of time.As the time went on,the correlation gradually decreased.4.The S100 B levels of cerebrospinal fluid and serum increased rapidly in 24 h after injury,and then began to fall,the severe group dropped significantly,the moderate group dropped slowly,then entered a relatively stable period in fifth days after injury.But the S100 B levels in the severegroup throughout higher than the moderate group.5.The higher S100 B level, the worse prognosis.6.The first day after injury,the S100 B levels of cerebrospinal fluid and serum in 1-2score group and 3-4 score group were significant higher than those of 5 score group.Conclusion As the specific marker protein of central nervous system, the S100 B levers of cerebrospinal fluid and serum decreased gradually along with the time after TBI. In the early stage after injury,the S100 B levels of cerebrospinal fluid and serum exist correlation in a certain period of time.But as the time went on, the correlation gradually decreased.By detecting the S100 B levels within a certain period of time in cerebrospinal fluid and serum of patients with TBI,we can predict changes of disease and prognosis.
