Investigation Of Correlation Of Serum Galectin-3, Homo Cysteine, High Sensitivity C-reactive Protein With Atrial Structural Remodeling Of Atrial Fibrillation

Objective:In this study, we investigate that the role of galactoselectin-3(Galectin-3), high sensitive C-reactive protein(hs-CRP) and homo cysteine(HCY) play in atrial structural remodeling via their correlation with left atrial volume index(LAVI). To promote better understanding of pathogenesis and risk factors of atrial fibrillation, provide more ideas for the prevention and treatment of atrial fibrillation in clinic. Methods:We included 107 AF patients(51 male and 56 female, mean age 65.2±8.6) from the second affiliated hospital of Nanchang university between Sep,2013 and Sep,2014, including 61 persistent AF and 46 paroxysmal AF. At the same time, 40 sinus rhythm( basic date similar to the AF group)were included as controls. Baseline characteristics were recorded. Fasting blood were siphoned in the second morning to detect serum levels of hepatorenal function, glucose, blood lipid, thyroid function, BNP, etc. All patients underwent electrocardiogram and 24 hour dynamic electrocardiogram. LAD, LVEF, LAV, etc. were calculated by echocardiographic method and LAVI was calculated. Galectin-3 and HCY level were measured by ELISA, hs-CRP was measured by immunoturbidimetry. Patients were classified into three groups(persistent AF group, paroxysmal AF group, controls). The comparisons about Galectin-3, HCY, hs-CRP were performed among three groups. Pearson correlation analysis, Multiple linear regression analysis and logistic regression analysis were used to analyze the relationship among Galectin-3, HCY, hs-CRP and LAVI. Results:1 、 Serum Galectin-3 in persistent AF group(1567.1±185.3 pg/ml) was significantly higher than these in paroxysmal AF group(1482.5±107.3 pg/ml)and controls(1389.4±121.2 pg/ml)(P<0.05), and so was in paroxysmal AF group than this in controls(P<0.05); serum hs-CRP in persistent AF group(3.85±3.01mg/L) was significantly higher than these in paroxysmal AF group(2.84±2.21mg/L) and controls s(1.71±1.40mg/L)(P<0.05), and so was in paroxysmal AF group than this in controls(P<0.05); serum HCY in persistent AF group(12.12±4.40μmol/L) was higher than these in paroxysmal AF group(10.31±3.41μmol/L) and controls(8.36±2.91μmol/L)(P<0.05), and so was in paroxysmal AF group than this in controls(P<0.05).2、Left volume index in persistent AF group(40.90±7.87ml/m2) was significantly higher than these in paroxysmal AF group(35.48±7.20ml/m2) and controls(31.39±4.51ml/m2)(P<0.05), and so was in paroxysmal AF group than controls(P<0.05). In Pearson correlation analyses, Galectin-3(r=0.618, P<0.01), hs-CRP(r=0.602, P<0.01), HCY(r=0.348, P<0.01) showed a strongly positive correlation with LAVI. Multiple linear regression analysis showed that LAVI was linearly associated with LAD, Gallectin-3, hs-CRP and atrial fibrillation.3、Logistic regression analyses showed that Galectin-3, hs-CRP, age, and LAVI were associated with atrial fibrillation. Conclusion:1、Serum level of Galectin-3, hs-CRP, HCY in persistent AF were higher than these in paroxysmal AF group and controls, and so was in paroxysmal AF group than these in controls;2、Galectin-3、hs-CRP showed a strongly positive correlation with LAVI in AF patients, which indicated that atrial fibrosis and inflammation take part in the procedure of atrial structural remodeling,and play critical role in the progression of AF;3、HCY showed a positive correlation with LAVI in AF patients,indicated that oxidative stress may participate in atrial structural remodeling.
