Objective:To explore the dynamic and static balance training with the patients of Chronic Chronic ankle instability(CAI) in the diagnosis and treatment research. Methods:Collectting 32 Chronic Ankle Instability patients and 30 patients without ankle sprains history. All subjects received CAIT and Pro-kin balance instrument test 3 times. The A and B who have skilled operation method of balance instrument. At the same time, the 62 cases of the subjects accepted the ankle proprioception pre-assessment the Pro- kin balance instrument multi-axis ontology sense evaluation and Cumberland ankle instability tool evaluation questionnaire scores respectively by the 2 therapist. Therapist evaluate 62 cases to evaluate one ankle proprioception subjects again in a same week with different time period. 62 cases of three evaluation index of the subjects by the ICC balance instrument reliability test. And the 32 results of CAIT and Pro-kin balance instrumen analyzed by Pearson correlation analysis rank correlation coefficient to test the validity of Pro-Kin balance instrument. Independend samples t-test was used to test the sensitivity of Pro-kin balance instrument. The 32 patients with CAI were equally divided into control group and treatment group by their willingness. The control group of 14 patients, to guide home by ankle pump contraction training;Treatment group of 18 patients, accepted ankle pump contraction training and dynamic and static balance training both. Treatment time is 4 weeks, 2 groups of patients in the David kotok ankle instability evaluation questionnaire scores, dynamic and static balance testing index(average trajectory error, the average load power error) were compered respectively before and after treatment. Results:The reliability between measurer was 0.788 ~ 0.961 and 95 % credibility intervals were centered. The multi-axis proprioception assessment index of Pro-Kin balance instrument showed a moderate negative correlation with CAIT(r=ï¹£0.421, ï¹£0.419 P<0.05); there was no correlation between preliminary assessment of proprioception and CAIT(r=ï¹£0.003-ï¹£0.164 P<0.05).The ICC of Pro-Kin balance instrument was 0.831~0.959 and 95% credibility intervals were centered. There were significant differences in multi-axis proprioception assessment index of Pro-Kin balance instrument test results between the two groups(P<0.05). There was no significant differences among the CAIT score, ATE and the average load power error after 4 weeks in control group(P>0.05). In the treatment group, the ATE and the average load power error were significantly lower after treatment, the CAIT score was significantly increased(P<0.01). The difference of CAIT score, ATE and the average load power error were statistically significant between the control group and treatment group after treatment(P<0.05). Conclusions:1. The Pro-Kin balance instrument showed a high reliability in evaluating the ankle stability of chronic ankle instability. 2.The multi-axis proprioception assessment index of Pro-Kin balance instrument can be used to evaluate the ankle stability of chronic ankle instability, and showed a high sensitivity.3.Dynamic and static balance training can significantly improve proprioception and stability in patients with chronic ankle instability.
