Objective: To explore the clinical application value of MSCT perfusion imaging in gastric cancer by comparing the differences of the perfusion parameters between gastric cancer and normal controls(NC),and among various cell differentiation degree of gastric cancer.Materials and Methods: Fifty patients with gastric cancer proved by pathology and twenty cases with normal controls(NC) from September 2014 to May 2015 were collected,stomach CT perfusion imaging were performed in seventy cases. Subject to be fasting 8-12 h or more,and were injected 654-210 mg at 10~ 15 min before the scanning, and were asked to drink water800-1200m1 at 5min before the scanning. Breath-hold training exercises were performed before the examination,all subjects were taken supine position and placed abdominal belt,using Siemens Sensation 64-VCT for body perfusion scanning. According to the abdominal plain scan image, determine 6 target layers base on gasric cancer center, Z axis scan range: 3cm, 120 k V/100 m As,thickness:4.8mm,512×512 pixels,small field of view(FOV).All cases were injected contrast agent(Ioversol 320 mg I/m L), the injection rate was5ml/s, the amount of contrast agent was 50 m L,and added to inject 12 m L normal saline,delay time:7s, scan time:30s.The CT perfusion scan data were processed on Syngo multi-modality workplace by CT perfusion service software package with deconvolution method. To select region of interest(ROI) at the maximum tumour slice or the most thick slice of NC.Theblood flow(BF), blood volume(BV), mean transmit time(MTT) and permeability surface(PS) mean values were measured and statistically analyzed by SPSSl7.0 statistical software. To compare the differences among various normal stomach wall,various gastric cancer location,various cell differentiation degree of gastric cancer,and compare the difference between gastric cancer and NC.P<0.05,the differences have statistic significance.Results: 1. CT perfusion color distribution of normal gastric tissue with different parts(lesser curvature of stomach, greater curvature of stomach,gastric antrum) was homogeneous.The BF,BV,MTT and PS mean values of normal controls were 75.07? 17.08ml/( min.100g), 7.95 ? 1.81 ml/100 g,16.67?2.40 sec, 7.98?1.30 ml/(min.100g). 2. CT perfusion color distribution of gastric cancer with different parts(center of carcinoma, surroundingof carcinoma, relevant normal regions) was different.The BF, BV and PS values were decreasing from center of carcinoma to relevant normal regions,but MTT value was increasing, the BF, BV and MTT values showed no difference between center of carcinoma and surroundingof carcinoma(P>0.05),but it showed obvious difference between center of carcinoma or surroundingof carcinoma and relevant normal regions(P<0.01),PS value showed obvious difference among three parts(P<0.01).The BF,BV,MTT,PS mean values for center of carcinoma were 101.28?48.01 ml/( min.100g), 16.09 ? 6.23 ml/100 g, 9.95 ? 4.99 sec, 33.00 ? 23.27 ml/( min.100g). 3.Comparing with normal controls(NC),the BF,BV and PS values of gastric cancer increased and MTT value decreased,CT perfusion four parameters values showed obvious difference between the two groups(P<0.01). 4. CT perfusion parameters values of gastric cancer were related to cell differentiation degree, the BF, BV and PS values were increasing from high differentiation to low differentiation, the BF, BV and PSvalues showed difference between high differentiation and medium or low differentiation group(P<0.05),but MTT value showed no difference(P>0.05).Conclusion: 1.Gastric cancer with CT perfusion imaging compared with normal controls(NC), it shows that the BF, BV, PS values increase and MTT value decreases. 2.The worse gastric cancer cell differentiation degree is,the higher the BF, BV, PS values are,but the MTT value shows no relationship. 3.The BF, BV, PS values may be assessment index in the malignant degree of stomach tumors.
