Object:Comparing the differential expression of(Procalcitonin, PCT) and analyzing the possible correlation between expression of PCT and inflammatory factors of different severity of diabetes(Diabetes Mellitus, DM) to investigate the possible values of PCT for diabetes condition assessment.Method:876 diabetic patients are choosed for the study to investigate the PCT level of different infection stages. To evaluate the correlation between expression of the disease prompted index and the PCT level, to analyses the relationship between PCT expression level and DM inflammatory factors(IL-6﹑TNF-α﹑INF-γ). The severity of acute complication of diabetes is assesed by APACHE II, the early and chronic complications of diabetes severity is assesed by Fasting blood glucose(FBG),Hemoglobin A1c(HbA1c) and Urinary micro Albumin(Um Alb).Result:1. The expression of PCT in DM increased with the severity of the disease.The level of PCT expression in DM patients was higher than that in normal healthy people, and the level of PCT in patients with acute complications was the highest, while the PCT level in early DM group was the lowest. TNF- increased with the severity of the disease, and other inflammatory index PCT, IL-6 in the early stage and chronic complications of DM and chronic complications of infection and local infection of inflammation showed up, and in the degree of chronic complications and systemic infection of inflammatory DM are not obvious changes in the trend.2. PCT expression level and the severity of the disease in DMThe expression levels of PCT in DM were higher than those in healthy subjects(0.1 + 0.05ng/ml). The correlation between DM and PCT, TNF-, IL-6 and PCT were found in DM and DM patients with DM and HbA1c%/, while FPG and INF- were lower than those in normal subjects(8.31 + 2.87pg/ml) in all patients. Correlation analysis showed that PCT, TNF-, IL-6, INF- and DM showed different degree of correlation. PCT was correlated with TNF-, INF-, INF-, IL-6, PCT, IL-6, PCT and TNF-, but the correlation was negative, but the correlation was statistically analyzed.n Hb A1c%/ FPG grading patients with DM in early stage without complications and chronic complications, Hb A1c%/ FPG were associated with disease classification, PCT ﹑ TNF-α ﹑ IL-6 ﹑ INF-γ showed positive correlation with disease classification, and TNF-α ﹑ IL-6 ﹑ INF-γ showed positive correlation. INF- showed negative correlation. PCT showed a positive correlation in local inflammation and systemic inflammation in patients with bacterial infection.PCT﹑TNF-α﹑IL-6﹑INF-γ of patients with APACHEII score were associated with DM score, while the correlation between <15 and >15 was poor.ConclusionThe differences of PCT express levels among the DM patients with different conditions is the reaction of the severity, PCT possibly has got full response capability for DM inflammation.
