Clinical Analysis Of Febuxostat In The Treatment Of Gout With Hyperlipidemia

Objective:This study was to investigate the relationship between serum uric acid(UA) and blood lipid levels in patients with gout,to study the urate-lowering efficacy and safety of febuxostat for patients with hyperlipidemia and gout and to further explore the lowering blood lipid effect of febuxostat.To find new treatment strategies for patients with hyperlipidemia and gout.Methods:150 cases of gout patients were chosen as gout group.30 males with routine physical examination were chosen as control group. The incidence of hyperlipidemia,serum uric acid and lipid level of two objects were compared between gout and control groups. 118 patients with hyperlipidemia were randomly divided into 3treatment groups, all of which were treated with lowering uric acid for 12 weeks. 3treatment group were febuxostat 40 mg group(febuxostat 40mg/d,n=40),febuxostat80 mg group(febuxostat 80 mg/d,n=38), allopurinol group,( allopurinol 300mg/d,n=40).The serum UA and lipid levels of objects before and after treatment in three groups and control group were compared. The occurrence of attack gout flares and adverse reactions were recorded.Results:The incidence of hyperlipidemia in gout group was higher than that in control group;gout group had higher levels of serum UA, triglycerides(TG), total cholesterol(TC), low density lipoprotein(LDL)( P ﹤ 0. 05).Compared with before treatment,the UA level was decreased after treatment in febuxostat 40 mg group,80mg group and allopurinol group,the difference was statistically significant(P < 0.05);There was no statistically significant difference between control group and febuxostat80 mg group(P>0.05). There was no significant difference between the 3 treatment groups before and after treatment of serum uric acid levels(P > 0.05).Compared with before treatment, The level of high density lipoprotein(HDL) after treatment in febuxostat 40 mg group increased(P< 0.05);Compared with control group,the level of HDL of patients in febuxostat 40 mg group after treatment had no significant difference(P> 0.05).The level of TG,TC and LDL of patients after treatment in febuxostat 80 mg group were all lower and HDL level was higher compared with before treatment(P< 0.05);Compared with control group, the level of TC, HDL, LDL and TG of patients in febuxostat 80 mg group after treatment had no significant difference(P > 0.05).The levels of TG, TC,HDL and LDL in allopurinol group after treatment changed little compared with before treatment, the difference was not statistically significant(P> 0.05).3 treatment groups during the treatment all attacked gout flares, but there was no statistical significance(P > 0.05), the incidence of adverse reactions in the 3 treatment groups was not statistically significant(P > 0.05).Conclusions:The patients with gout are often combined with dyslipidemia,elevated serum triglyceride, total cholesterol, low density lipoprotein cholesterol levels are mainly performances For patients with hyperlipidemia and gout,Febuxostat and allopurinol may reduce the uric acid significantly.Febuxostat may lower the levels of lipids and febuxostat 80mg/d may reduce the level of TG, LDL, TC and increase the level of HDL.Gout flares and adverse reactions were similar for febuxostat and allopurinol.
