Study On The Status And Countermeasure Of Medical Service And Operation Quality Of Country Public Hospitals In Jilin Province

PurposesCountry public hospitals is an important part of Chinese health care system, and the medical quality is the key to the development of medical institutions. It is not only the core of hospital management, but can also guarantee medical safety. This stud y focused on the quality of medical service and operation in country public hospitals. The overall objective of this study is to analyze the changes of the medical quality of country public hospitals in Jilin Province from the year 2012-2014, and to provide evidence and propose strategies to improve the overall service capacity of country hospitals. MethodsQuantitative and qualitative research methods were both used in this study. The main source of information about this study was from three channels:(1)literature. The review was collected from China National Knowledge, CNKI.(2) Focused group discussion. Focus groups included the hospital management experts, head of the hospital, clinical experts, etc. The qualitative data are used to do deep analysis of the main problems of the medical quality.(3) Quantitative survey. Considering the level of economic and willingness to be investigated, we chose 26 county-level public hospitals from 9 cities in Jilin Province. Medical quality data was collected by questionnaire to describing the changes of the medical quality of county public hospitals in Jilin Province during 2012-2014. ResultsThe medical care quality of country public hospitals in Jilin Province has been improved in nearly three years, but the problems are existed as well.1) Due to the lack of the qualified medical talents, there are many treatment programs can not be carried out in country public hospitals. That limited medical service capacity of country public hospital, but also increased the difficulty of treatment. 2) The hospital information at low level and the process was slow; 3) Prescription reviews was on an overall upward trend in country public hospital, but the growth was slowly, and there was a certain gap with the national standards; 4) The proportion of cesarean section in Jilin country public hospital was too high; 5) The assets and liabilities of country public hospital were too high, in some areas expenditure was greater than income. That increased the financial burden on hospitals; 6) Country public hospital medical costs continued to grow rapidly, increased the burden on patients. 7) Drug revenues accounted for a high proportion of general income of country public hospitals. Supporting medicine with drugs also existed. China has yet to develop a special country-level public hospital medical quality indicators evaluation system and standards, the country-level public hospital medical quality is no reference value. ConclusionThere is no specific national medical quality indicators and evaluation criteria detailed metrics for country-level public hospitals interpretation at present. And there is no clear standard for country level public hospitals’ own business. There are many problems in the country level public hospital in Jilin province, high debt, unreasonable income structure, the rapid growth of medical costs. Due to the talent shortage, country public hospitals can not carry out individual treatment programs, and the allocation of resources is not reasonable. Therefore, it is essential to increase the support of public hospitals at the country level.
