Preventive Effect Of Augmented Jade Screen Decoction On Nipple Cracks In Breastfeeding Women

ObjectiveTo evaluate the effects and effectiveness of augmented Jade Screen decoction (astragalus root, atractylis ovata, radix sileris, the root bark of white mulberry, Liquoric Root, etc. were included) on preventing sore and crack of nipples during breastfeeding in comparison with the application of expressed colostrums or plain water in Chinese Breastfeeding Women.MethodsUsing the randomized control design,300 pregnant women were enrolled and randomized into control group, pre and postnatal intervention group and postnatal intervention group. Women in pre and postnatal intervention group were treated with augmented Jade Screen decoction twice a day on the morning and night started from 37 gestation weeks till 7 days postpartum while the postnatal intervention group were started from 2 hours after delivery till 7 days postpartum Women in control group were treated with plain water or colostrums if it is available. Breastfeeding skills were implemented under the supervision of midwives. Nipple trauma and pain intensity were evaluated by nipple trauma score and visual analog scale (VAS) every day after 12 hours postpartum. Baby feeding condition were followed up by phone calls 6 months after delivery.ResultsAfter 21 cases who were not willing to accept the follow-up excluded from the patients,279 cases were included in the final analysis,90 cases in the control group,94 cases in the postpartum intervention and 95 cases in the pre and postnatal intervention group. The incidence of cracked nipple were 10.53%,21.28% and 25.56% in the pre and postnatal intervention group and control group respectively with significant difference (P<0.05). Further pairwise comparison found the in the incidence of cracked nipple in the pre and postnatal intervention group was significantly lower than the control group (P= 0.008), and significantly lower than postnatal intervention group at the same time (P= 0.043); while there was no significant difference between control group and intervention postpartum intervention group in the incidence of cracked nipple (P= 0.493). Among the 23 cases of chapped nipple in the control group,4 cases (17.39%) were mild chapped, while 2 cases (8.70%) were severe. Among the 20 cases of postpartum intervention group,3 cases (15.00%) were mild chapped, while 1 case (5.00%) was severe. And Among the 10 cases of pre and postnatal intervention group, mild and severe chapped were 2 cases (20.00%) and 0 case respectively. There was no significant difference between three groups in severity. There were 17(17.89%)cases in pre and postnatal intervention group,30 cases (31.91%) in postpartum intervention group and 35 cases (38.89%) in control group suffered from severe nipple pain. And the difference between the three groups was significant(P<0.05).A further intercomparison was made in these groups and showed the nipple pain in the pre and postnatal intervention group was significantly milder than control group (P=0.001) postpartum intervention group (P=0.026).While there was no significant difference between control group and postpartum intervention group (P=0.322).ConclusionsThe research indicated that the use of augmented jade screen decoction from 37 gestation weeks to 7 days post partum is effective on preventing nipple cracks and relieve nipple pain in Chinese Breastfeeding Women. And it seems that treatment indirectly increase the rate of pure breastfeeding in 6 months.
