| Objective:This study was designed to analyze the characteristics and features of hippocampal volume of AD, MCI patients in our hospital and the relationship with MTA-scale, Neuropsychological tests scores.In order to have a better understanding the hippocampal structural changes and the relationship with cognitive function of AD,MCI patients,and have beneficial to clinical early diagnosis and treatment.Method:Select 15 patients of AD, 22 patients of MCI, and at the same time select 20 healthy people as control whose age, sex, educational level is matched,who visit to First Hospital of Jilin University from January 2015 to February 2016. All of them need do the MRI and neuropsychological tests.Useing the Roi Assist401 software outline the bilateral hippocampal ROI in the 3D-MPRAGE manually in evey level,and automatically generate the volume of each area,and then multiplied by the thickness for the volume. Use the SPSS software analyze the results.Results:Enrolled 57 cases including AD patients, MCI patients and CTRL group.The hippocampal volume of the three groups are CTRL> MCI > AD,and the difference between each of two groups were statistically significant(p<0.05).Bilateral hippocampal volume were statistically significant with gender and age,but no significant correlation with education level(p> 0.05).The hippocampal volume of males was more than females(r = 0.413, p<0.05);Bilateral hippocampal volume were positively correlated with MMSE, MOCA, AVLT, BNT, AHFT,with TMT was negatively correlated(p<0.05); Left and Right of the MTA-Scale and hippocampal volume were significantly negative Related,which is mean that the higher of the MTA grade’s score,the hippocampal atrophy is more significant(left r =-0.684, p <0.05,right side r =-0.588, p <0.05). The difference of Hippocampus volume and MTA-scale between AD with MCI and CTRL was statistically significant(p<0.05),and it was statistically significant(p <0.05) between CTRL and MCI,but it was no significant difference between MTA-scale and CTRL(p>0.05).Conclusion:1, The hippocampal volume of AD, MCI are reduced.Compared with MCI, The hippocampal volume of AD group shrinking significantly; 2, Hippocampal volume is significantly related with gender, age.the hippocampal volume of men is bigger than women. Hippocampal volume has no significant correlation with the level of education, the diagnosis of AD and MCI have better stability. 3,.Hippocampal volume and Neuropsychological evaluation results have a high correlation.the more severe atrophy, the lower score of the neuropsychological tests, the more severe cognitive dysfunction; 4, Hippocampal volume and MTA-scale was significantly related, which could provide confidence for the diagnosis of AD, but the MTA-scale could not recognize MCI patients well, and the diagnostic value of hippocampal volume for AD, MCI is superior to MTA classification. |
