Objective: To improve hemiplegic stroke patients with prognosis problem, increase stroke patients with hemiplegia activities of daily living, limb movement function, patient compliance, implement Transitional Care intervention on stroke patients with hemiplegia. To provide evidence for the future of Transitional Care mode.Methods: 100 cases of elderly patients with cerebral ischemic stroke in the department of neurology in some hospital in Changchun from Feb, 2014 to Aug,2015 were chosen and divided into the intervention group and the control group using the method of Random Numbers Table,50 cases each group. The control group was treated with routine nursing care druring the period of hpspitalization, and received routione discharge instruction by the responsible nurse. The exprienmental group was also treated with routine nursing care, and received transitional care strategies three days before discharge performed by the responsible nurse and continued to three month after the discharge. The basic information from the patients in two groups were collected at discharge, after 6 weeks, 3 months, related materials were gathered again and comparatively analyzed. Comparison of the activities of daily living, limb movement function, patient compliance, patient satisfaction with discharge care, laboratory indexes in two groups is different or not,and the difference is statistically significant or not. Research data are managed by SPSS17.0 software package for statistical process, the methods are adopted such as descriptive statistics, chisquare test, t-test, etc.Results: 1. The difference of the general information between the intervention and control group was not statistically significant(P>0.05). 2. After six weeks of transitional care intervention, the intervention group had better outcome in activities of daily living, patient compliance except reasonable diet and treatment compliance and patient satisfaction with discharge care than the control group. The difference was statistically significant(P<0.05). There were no significant differences in movement function, patient compliance of reasonable diet and medicin, laboratory indexes.(P> 0.05). 3. After three months of transitional care intervention, the indicators in the intervention group such as the activities of daily living, limb movement function, patient compliance, patient satisfaction with discharge care and laboratory indexes were significantly improved than the control group,and there were statistical meaning in the difference(P<0.05).Conclusions: 1 Transitional Care intervention could effectively improve the ability of daily life of stroke patients. 2 Transitional Care intervention could effectively improve the limb function of stroke patients.3 Transitional Care intervention could effectively improve the compliance of stroke patients. 4 Transitional Care intervention could effectively improve the laboratory indexes of stroke patients. 5 Transitional Care intervention could effectively improve patient satisfaction with discharge care of stroke patients.
