Background Premature Rupture of membranes(PROM) is the rupture of membranes occurs before labor, and is one of the perinatal serious complications. The trend of PROM is rising recently, and is greatly increasing the mother’s morbidity and mortality in the perinatal period. Premature rupture of membrane can cause adverse pregnancy outcomes such as amniotic fluid reduction, infection, premature labor, umbilical cord prolapse and compression, placental abruption, fetal distress, and so on, which is dengerous for maternal and fetal. There are large amounts of studies related to PROM, howere the mechnism of its pathogenesis is still unclear. Our study focused on the expression of tumor necrosis factor related apoptosis inducing ligand(TRAIL) and nuclear factor kappa B predominate(nf-kappa B) in the membrane of the pregnant women with premature rupture of membranes, to study the mechnism of PROM, in order to give a way on theraputic method.Objective The aim of our study is to detect the expression of TRAIL and nf-kappa B of PROM patient. Then test the relationship between TRAIL and nf-kappa B in PROM pregnant, to reveal the mechnism of PROM.Materials and methods1 Study Object We selected 60 cases caesarean section patients, including 30 full-term patients with PROM(experimental group) and 30 cases with normal pregnant women(control group), which were collected in the third affiliated hospital of Zhengzhou university from June 2014 to November 2014. Experimental group patients aged from 21 to 36 years old, gestational aged from 37 to 41 weeks; control group patients aged 20 to 37 years old, gestational aged 37 to 41 weeks. The including principles are single birth with the position of the head, primipara, without other maternal disease, infectious diseases, and other obstetric complications etc, then cesarean section indications are contracted pelvis, abnormal position and cephalopelvic disproportion, etc. 2 Study Methods(1) Immunohistochemistry method was used to detect the protein expression of TRAIL and NF-κB in the membrane tissue after delivery.(2) RT-PCR method was used to detect the m RNA expression of TRAIL and NF-κB in the membrane tissue after delivery. 3 Statistical Methods SPSS 19.0 software was carried on the statistical analysis of all the data, and 2- Δ Δ Ct method was adopted to analyse the data from Real-time fluorescent quantitative PCR assays. Measurement data were presented in the form of a mean ± standard Deviation, and t-test was used to compare the two independent samples; then the Chi-square test was used to compare enumeration data; Pearson correlation analysis was used to analysis correlation, inspection level α = 0.05, tatistical significance was considered when P < 0.05.Results1 Comparison of genral conditions in the two groups of pregnant women Experimental group of pregnant women with an average age(27.63 ± 2.93) years and with the average gestational age(38.87 ± 1.28) weeks; while control group of pregnant women with an average age(27.43 ± 1.09) years and with the average gestational age(38.93 ± 1.34) weeks. Pregnant women age and gestational age between the two groups were compared, the differences had no statistical significance(P > 0.05). 2 The position and the level of TRAIL and NF-κB protein expression in experimental group and control group TRAIL mainly expressed in the membranes cytoplasm of chorion, amniotic epithelial cells, the expression rate of chorionic was higher than amniotic membrane; NF-κB was expressed in the membranes cytoplasm and nucleus of chorion and amniotic epithelial cells, located on nucleus in the experimental group, while the control group mainly located on the cytoplasm. The positive expression of TRAIL and NF-κB in the experimental group respectively were 26 cases(87%) and 27 cases(90%), while the control group respectively were 17 cases(57%) and 18 cases(60%), both the membranes issues positive expression rate in experimental group is higher than the control group, the differences had statistical significance(P < 0.05). 3 The TRAIL m RNA and NF-κB m RNA expression in the two groups of experimental group and control group TRAIL m RNA and NF-κB m RNA were detected in the two groups.The expression level of TRAIL m RNA and NF-κB m RNA in the experimental group respectively were 1.63 ± 0.19 and 3.72 ± 0.19, while the control group respectively were 1.00 ± 0.16 and 2.00 ± 0.15; the differences between the two groups had statistical significance(P < 0.05). 4 The expression correlation between TRAIL and NF-κB in the fetal membrane issues The expression level of TRAIL had correlated with the NF-κB in the membrane of PROM(r = 0.875, P < 0.05).ConclusionsThe high expression of TRAIL and NF-κB of pregnant women membranes organizationins was involved in the disease process and promoted the occurrences about PROM, closely related with premature rupture of membranes. The expression between TRAIL and NF-κB in the fetal membrane issues had significantly positively correlation, which shows the TRAIL and NF-κB may be through a mechanism together regulate and promote the occurrence of premature rupture of membranes.
