| Objective: Chronic pulmonary heart disease(CPHD) secondary to chronic obstructive pulmonary disease(COPD) in clinical work patients with Pulmonary thromboembolism(PTE), increase the risk of death, a serious impact on the prognosis of patients with PTE,so I want to explore the clinical characteristics of CPHD patients with PTE.Methods: A retrospective analysis in January 2012 and 2015 December in our hospital inpatients of the Department of respiratory medicine of 157 patients with CPHD patients divided as case group(74 cases) and(CPHD+PTE group), and the control group(n = 83)(simple CPHD group), to the patient’s general situation, clinical symptoms, signs and auxiliary examination results were compared.Results: Two groups of patients with smoking history,gender, age, cough, expectoration, course of disease, difficulty in breathing, shortness of breath, chest tightness, chest pain, hemoptysis, lung wet rales, double lower limbs edema generally, symptoms and characteristics of no statistical significance(P > 0.05). The incidence of syncope in the higher CPHD+ PTE group pure CPHD group, the difference is statistically significant(P < 0.05); admission BNP and D- dimer body, protein fiber, the electrocardiogram(ECG), echocardiography measured cardiovascular parameters and other auxiliary examination results between the two groups no statistical difference(P >0.05). Double lower limb venous ultrasound and the incidence of type I respiratory failure,P(A-a)O2 between the index had significant difference(P<0.05).Conclusion: Chronic pulmonary heart disease and chronic pulmonary heart disease complicated with pulmonary embolism clinical symptoms are difficult to distinguish when the CPHD patients with syncope and venous thrombosis of the lower limbs, with type I respiratoryfailure and P(A-O) 2 value increased to improve the examination of CT pulmonary angiography(CTPA) further determine whether complicated with PTE, timely and effective treatment, help to improve the prognosis of CPHD complicated with PTE patients. |
