Objective: Based on the observation of Wnt/β-catenin、GSK-3β on the expression of vascular disease in type 2 diabetes mellitus mechanism, And the intima-media Danzhijiangtang Capsule in patients with vascular and carotid artery thickness(IMT)and plaque area. At the same time to explore the GSK-3βand Wnt/β-catenin gene from the gene expression level of protective effect on carotid artery in diabetic rats and the mechanism 。Methods:1. Clinical Research:102 cases of patients with type 2 diabetes mellitus,Random grouping,The control group of 51 people,and the treatment group of 51 people,All patients were given basic drugs(conventional oral hypoglycemic drugs) treatment, The treatment group was treated with drug + Danzhijiangtang Capsule for 16 weeks. Before and after treatment were calculated for the total efficiency of the two groups of patients, the level of blood glucose,glycosylated hemoglobin(Hb A1c),insulin,blood lipid,serum β-catenin,GSK-3 β and observe changes of carotid artery IMT and plaque score before and after treatment.2. Experiment:70 healthy male SD rats after 1 weeks of feeding were randomly divided into control group 10 and experimental group 60. The end of the blank group were fed with common feed to experiment. Experimental groups according to 6.5 x 105u/kg one times of injection of vitamin D3 and fed with high fat diet and contributed to the rat model of atherosclerosis was 4 weeks after intraperitoneal injection of small dose of streptozotocin(STZ) of streptozotocin(STZ) to induce type 2 diabetic patients with atherosclerosis rat model. Will make a successful model of 57 rats were randomly divided into 4 groups: control group(n = 12),low dose group 15,middle dose group 15rats) and high dose group(n=15), the blank group and the control group be physiological saline by gavage, low, medium and high dose group gave a certain percentage of Danzhi Jiangtang Capsule by gavage, intragastric administration was continued for 12 weeks. Respectively before and after treatment in observation groups rats behavior,weight,mental state and the measurement of FPG,2h PG,TC and TG,after 17 weeks killed rats and detection of serum β-catenin and GSK-3βcontent. The carotid artery was separated from abdominal aorta blood was used to detect the Wnt /β-catenin、GSK-3βm RNA expression levels.Results:1. Clinical Research:Clinical observation showed that after 16 weeks of treatment with Danzhi Jiangtang Capsule,in the control group,the total efficiency was 64.71%,the treatment group84.31%,treatment group curative effect surpasses the control group(P < 0.05),Patients with fatigue, fatigue, dry mouth and throat, dizziness headache, Skin armour fault and other main symptoms are somewhat improved after treatment, respectively, before treatment, the difference was statistically significant(P <0.05);The two groups before and after treatment of TCM syndrome integral have significant difference(P < 0.01),the treatment group and the control group compared to the difference is statistically significant(P < 0.01).After treatment, two groups of FPG, 2h PG, Hb A1 c, TC, TG, LDL, HDL, IMT and carotid plaque area level improved(P<0.05 or P<0.01), 2h PG, Hb A1 c, TG and LDL groups decreased with statistical significance(P<0.05 or P<0.01), the treatment group improved better than the control group IMT(P<0.05), but no significant improvement in plaque area(P ﹥ 0.05). After treatment; serum β-catenin level was lower than the control group(P<0.01), serum GSK-3 β was lower than the control group, the difference was statistically significant(P<0.05)2. Experiments:Animal experimental results show that the use of Danzhijiangtang Capsule filling,low, middle and high dose group rats weight significantly increased(P<0.01), low,medium and high dose group compared with the control group FPG decreased significantly(P<0.01), TG, TC and LDL decreased, HDL increased, the difference was statistically significant(P<0.05 or P<0.01); After treatment, β-catenin and GSK-3βlevel and blank group compared to low dose group have significance(P<0.05), high dose group of β-catenin had significant difference(P<0.01), middle dose group of GSK-3β had statistically significant difference(P<0.01); low, medium and high dose group and the control group compared to a statistically significant difference(P<0.05 or P<0.01).after the treatment of β-catenin expression, compared with the control group,low dose group, significant(p<0.05), the high dose group had significant difference(P<0.01); low dose group was statistically significant compared with the high dose group(p<0.05); after the treatment of GSK-3βcompared with the blank group, medium and high dose group had significant difference(p<0.05), compared with the control group,low dose group differences(p<0.05), there was significant high dose group(p<0.01).Conclusion:Danzhijiangtang Capsule can significantly improve type 2 diabetic patients with clinical symptoms, in a certain extent improve or reverse the carotid arteriosclerosis. The mechanism may be: reduce blood sugar, improve insulin resistance, regulate lipid metabolism, improve vascular endothelial function, improve the serum levels of GSK-3β,reducing the levels of β-catenin accumulation,regulation of Wnt / β-catenin m RNA and GSK-3β m RNA expression, which plays an important role in vascular protection role.
