Objective To explore the value of three-dimensional reconstruction in the treatment of hilar cholangiocarcinoma. Methods The clinical data of 34 patients with hilar cholangiocarcinoma undergoing resection combined hepatectomy were collected. Three-dimensional surgical planning was applied to 15 cases(group A),while it was not applied to the rest 19 cases(group B). The data including the relevant surgical variables and postoperative complications were analyzed. The tumors of patients with hilar cholangiocarcinoma in group A were classified by the three-dimensional construction system, the differences between the actual and planned surgical results were analyzed. Then the total liver volume, pre-cut volume and remnant liver volume were measured according to the two-dimensional and 3D images.Results 1. The mean bleeding volume, operation duration and postoperative extubation time were (525.4±140.9)mL, (5.0±0.6)h and (8.9±3.2)d in group A, and were (676.3±185.2)mL, (5.8±1.0)h and (11.4±3.3)d in group B, the inter-group differences had statistical difference(P=0.014, P=0.020, P=0.037), but the inter-group rate of postoperative complications and hospital stays had no statistical difference(P>0.05).2. The accuracy of tumor classification by three-dimentional reconstruction system was 13/15. The mean liver resection volumes calculated by the two-dimensional techniques and three-dimensional reconstruction were (404.9±248.5)mL and (411.2±254.6)mL, with no significant difference(P>0.05). The actual liver resection volume was positively correlated with the three-dimensional results(r=0.999, P<0.05).Conclusion Three-dimensional reconstruction can exactly evaluate hilar cholangiocarcinoma before the operation, and it has a certain clinical significance in the surgical treatment.
