ObjectiveUsing the event related potential (ERPs), use the P300 and N400 experimental paradigm, evaluation cognitive function in patients with temporal lobe epilepsy with anterior temporal lobectomy and postoperative, analysis of the change characteristics of P300 and N400 in patients with temporal lobe epilepsy before and after anterior temporal lobectomy and, preliminary study existing cognitive dysfunction in patients with temporal lobe epilepsy, and then discusses the influence to cognitive function in patients with anterior temporal lobectomy.MethodsChoose 10 patients with anterior temporal lobe resection in nerve medical center in the people’s liberation army 174 hospital in Xiamen between December 2014 and December 2015. Preoperative Use 64 guide ERP record and analysis system of Genmany Brain Products to detection of P300 and N400; To location epileptogenic zone with magnetic resonance imaging (MRI), video electroencephalogram (video electroencephalography, VEEG) and positron emission tomography imaging (positron emission computerized tomography, PET).To location the epileptogenic zone more precise again,we use the (electrocorticogram, ECoG).Follow-up in postoperative,we testing P300 and N400 after 1 month,3 months,6 months.Results①Latency of P300 between epilepsy group with normal controls:using independent sample t test to get mean and standard deviation respectively.The results showed that the CZ and PZ of P300 latency extension of 42.54 ms and 46.53 ms compared with control group, with significant difference, P<0.05;②P300 latency and amplitude compared with pre-and-postoperative:after 1 month,3 months,6 months,P300 latency of CZ was shorten 24 ms,27.4 ms and 25.4 ms,with significant difference, especially 3 months and 6 months, difference has the extremely significant (P<0.01), after 1 month,with significant difference (P<0.05); And P300 latency of PZ in 1 month,3 months after surgery is not obvious difference, but in 6 months the P300 latency reduced 36.2 ms, with significant difference(P<0.05). The CZ and PZ’s P300 amplitude difference is not obvious, there was no significant pre-and-postoperative; ③ P300 latency and amplitude compared with pre-and-postoperative:N400 latency of PZ was no significant meaning compared with the preoperative.And amplitude of N400 was no significant meaning compared with the preoperative.ConclusionsDrugs refractory TLE have cognitive dysfunction,, patients’ clinical seizures under control after anterior temporal lobe resection.There were seizure free during the follow-up of 6 months, and at the same time the period of P300 were shorten compared with preoperative.That was showed the patients with brain to information processing enhancement; Incubation period For the latency of N400 has no obvious change,compared preoperative. |
