Idiopathic Membranous Nephropathy Clinical Treatment Plan Optimization Research

Objective: Observe three kinds of treatment for primary treatment clinical curative effect and safety of membranous nephropathy, provide reasonable primary membranous nephropathy, standardized, efficient and safe treatment.Methods: Using parallel control principle, prospective design, according to the inclusion criteria, 180 patients syndrome diagnosis standards are divided into:combine traditional Chinese and western medicine treatment group, western medicine control group, control group of traditional Chinese medicine and other three groups,each group of 60 cases. Foundation treatment: to give enough prednisone dragon, at the same time to treatment such as calcium, lipid-lowering, detumescence, banning the use of other drugs have an effect on proteinuria. Combine traditional Chinese and western medicine treatment group: basic treatment and cyclophosphamide combined traditional Chinese medicine(TCM) preparations. Western medicine control group:basic treated by cyclophosphamide. Chinese medicine control group: basic treatment of combined traditional Chinese medicine(TCM) preparations. Total 16 weeks treatment. Three groups were observed after treatment for 12 weeks, 16 weeks after the clinical curative effect of observation of combining traditional Chinese and western medicine treatment group and control group two groups of traditional Chinese medicine therapy 12 weeks, 16 weeks after the symptoms curative effect,routine urine test before and after treatment at the same time the main index, 24 hoursurinary protein quantitative(24 h UTP), plasma albumin, secondary index blood fat,liver and kidney functions, safety index using SPSS13.0 statistical software for all data for statistical analysis.Results: 1. The Chinese and western medicine treatment group total effective rate was 74.13%, the western medicine control group total effective rate was 70.17%,Chinese medicine control group total effective rate was 63.79%. The comparison group difference was statistically significant(P < 0.05). 2. The three groups after treatment from the previous 24 hours urinary protein, urinary protein quantitative reduction, plasma albumin level from the previous, more statistically significant(P <0.05). 3. The three groups(Alt, aspertate aminotransferase) in liver function tests, and renal function tests, serum creatinine, blood urea nitrogen) before and after treatment is no statistical significance(P > 0.05). 4. Combine traditional Chinese and western medicine treatment group and control group after treatment of traditional Chinese medicine can improve TCM symptoms and lower TCM symptom score, after treatment of TCM syndrome integral comparative differences between groups was statistically significant(P < 0.05), the combination of Chinese and western medicine treatment group part of TCM symptom improvement situation is better than that of traditional Chinese medicine control group(P < 0.05).Conclusion: combine traditional Chinese and western medicine scheme used in the treatment of idiopathic membranous nephropathy, which can improve TCM symptoms, reduce urinary protein, improve the level of plasma albumin or restore and protect renal function, and less side effects. With popularization and application value in clinic.
