| Purpose: Testing in patients with type 2 diabetes, we analyze the influence of self-efficacy and self-management level and the degree of psychological distress, stress on blood sugar of patients with type 2 diabetes. This study provide new ideas and theoretical guidance for the treatment of diabetes.Methods: Respondents can be divided into good blood sugar control and poor blood sugar control groups according to the patient’s blood sugar level. We use General situation questionnaire, Diabetes Distress Scale, Stress Response Questionnaire, the Self-efficacy for Diabetes and Diabetes self-management questionnaire to test the patients with type 2 diabetes form the first hospital affiliated to soochow university. We analyze the relationship between blood sugar levels and related factors using statistical methods including T test, correlation analysis, factor analysis, regression analysis method.Results: 1. T test results: Compared with good blood sugar control group, the score of diabetes mental pain and four dimensions(emotional burden, pain associated with the doctor, the pain associated with routine, interpersonal relationship related pain) in poor blood sugar control group were significantly higher(P(27)0.01). the score of stress reaction and two dimensions(emotional reactions and behavioral response) in poor blood sugar control group were significantly higher(P(27)0.01). the score of diabetes self-efficacy and self-management in poor blood sugar control group were significantly lower(P(27)0.05). Fasting blood sugar, diabetes pain and its each dimension, stress reaction and its each dimension(except the behavioral response) in high self-efficacy group was significantly lower than low self-efficacy group.2. Correlation analysis results: The FBG and age of the respondents were significantly positive correlation(P(27) 0.01).The FBG and income level, educational level of the respondents were significantly negative correlation(P(27) 0.05). And weight, gender, no significant correlation. The FBG and Diabetes mental pain(emotional burden, pain associated with the doctor, the pain associated with routine, interpersonal relationship related pain) of the respondents were significantly positive correlation(P(27) 0.05). The FBG and stress reaction(emotional reactions, physical reaction, behavioral response) of the respondents were significantly positive correlation(P(27)0.05). The FBG and Self-efficacy, diabetes self-management level were significantly negative correlation(P(27)0.05).The Diabetes psychological distress and self-efficacy, diabetes self-management level were significantly negative correlation(P(27)0.05).The Self-efficacy and stress reaction were significantly negative correlation(P(27)0.01). the Self-efficacy and self-management level were significantly positive correlation(P(27)0.001).3. The principal component factor analysis and regression analysis results: Five main factors can be extracted according to the principle of eigenvalues greater than 1,including Psychological distress factor(emotional burden, doctor related pain, pain associated with routine, interpersonal relationship related to the pain), stress factor(emotional reactions, physical reaction, behavioral response), social function factor(age, educational level, income level), Biological factors(Weight and gender), self-control factor(diabetes self-efficacy and self-management). Stepwise regression analysis results show that mental pain factor, stress factor, social function factor, self-control factor entered into the model. Mental pain factor and stress factor can positively predict FBG, the social function factor and self-control factor can negative predict FBG.Conclusion: 1. High age is a risk factor of blood sugar control, high cultural level and income level is the protective factors of blood sugar control for patients with type 2 diabetes, the social function factor can negative predict FBG.2. Mental pain and stress have important influence on high blood glucose values, mental pain factor and stress factor can positively predict FBG.3. High self-efficacy and self-management level is helpful to maintain blood glucose control in patients with type 2 diabetes, the social function factor and self-control factor can negative predict FBG. |
