Establishing And Application Of Nursing Adverse Events Reporting Information System In Hospital

Objective:(1)In order to collect, analysize, exchange and share maximum safety information,establish non-punitive and voluntary reporting system about care adverse event,establish a unified information platform,so that nursing management of adverse events would be more standardized and scientific,the utilization of information resources and the quality and efficiency ofcare would be improved, promoting patient safety, improving health carequality, reducing medical disputes.(2)Create an atmosphere of safety culture in the hospitals.(3)Provide a theoretical basis of the decision-making for the health administrative departments.Methods:(11)Developing care adverse events reporting form according to 5W1H(what\where\when\who\why\how) and root cause analysis of adverse events and related literature recommended by national patient safety agency and actual circumstances of hospital; (2)Developing reporting system of care adverse events and security risks of patients and related reward and punishment rules. (3)Statistics various types of care adverse events and safety risks of patients, discuss and analyze adverse events by hospital safety committee, summarize and educate related works, improve the level of nursing management.(4)Establish network reporting system of nursing adverse event in the internal LAN (Local Area Networks) of the hospital by information technology.Nurses log on to the system by the number of own or anonymous manners and report nursing adverse events.Results:(1)A year before and after the implementation of nursing adverse event reporting system, reporting nursing adverse events increased from 16 to 140, Errors increased from 16 to 58, non-errors increased from 0 to 82, adverse events categories increased froml 5 to 15; A year before and after the implementation of system, reporting rate of nursing adverse events in inpatients was significant differences (p< 0.05), emergency patients no significant difference (p> 0.05); (2) Adverse event network reporting system have characteristics with easy operation、confidentiality、fast delivery、feedback、 sharing of lessons、facilitate the statistical information and comprehensive understanding of adverse events, fully reflects science and human characteristics of the care and safety management.Conclusion:(1) Voluntary, non-punitive reporting system for nursing adverse events can establish safety culture in the hospital(2) Voluntary, non-punitive care hospital adverse event reporting system is the most important resources finding errors, can increase the number of nursing reported adverse events,reported types,reporting rates,reporting channel- s,non-nursing errors among reported adverse events, find the risk factors leading to errors before the occurrence of adverse events; (3) The amount of reported incidents does not mean that the degree of corporate security; (4) Adverse event network reporting system have characteristics with easy operation,confidentiality,fast delivery,feedback,sharing of lessons facilitate the statistical information and comprehensive understanding of adverse events, so that nursing managers fully understand situation of adverse events and promote the continuous improvement of nursing safety management; (5) Trough showing effect of experience and practical features in the process of establishing the system, it is useful to establishing, promoting and improving of medical adverse event reporting system of hospital in China.
