Objective:To investigate the value of peripheral blood of neutrophil-lymphocyte ratio in diagnosis of early-stage type 2 diabetic kidney disease.Methods:The 154 subjects were divided into three groups:A group (normal albuminuria group, stage Ⅰ~Ⅱ of diabetic kidney disease) (N=36),19 cases of male and 17 cases of female, B group (micro albuminuria group, stage Ⅲ of diabetic kidney disease) (N=55),34 cases of male and 21 cases of female and C group (the healthy control group) (N=63),31 cases of male and 32 cases of female. The diabetic kidney disease stage is divided according to the Mogensen staging. The numbers of neutrophil-lymphocyte ratio, white cell count, neutrophil count, lymphocyte count, monocytes count, C-reactive protein, lipid profile, fasting plasma glucose,24 hour micro albuminuria, as well as height, body weight, blood pressure were examined. Factors of early-stage diabetic kidney disease were calculated using variance and logistic regression analysis.Result:(1).The neutrophil-lymphocyte ratio, white cell count, neutrophil count, monocytes count, C-reactive protein in A group and B group were significantly higher than those of the healthy control group (P<0.01, respectively); (2).C-reactive protein in B group was significantly higher than A group (P<0.01); there was no difference in the neutrophil-lymphocyte ratio, white cell count, neutrophil count, monocytes count between A group and B group(P>0.05, respectively).(3).There was no difference in lymphocyte count in the three groups (P>0.05). (4).Logistic regression analysis showed that the risk factors of early-stage diabetic kidney disease include neutrophil-lymphocyte ratio, C-reactive protein, triglyceride, lipoprotein a. Receiver operating curve analysis suggested that the optimum neutrophil-lymphocyte ratio cutoff point for early-stage diabetic kidney disease was 1.990 with 90.5% specificity and 48.8% sensitivity.Conclusion:Increased neutrophil-lymphocyte ratio levels may be a predictive and reliable marker of diagnosis of early-stage type 2 diabetic kidney disease.
