Objective:To evaluate the effect of using temporary abdominal aortic balloon occlusion during open reduction and internal fixation of complex and fresh acetabular fractures for reduce hemorrhage. Analysis the related factors, such as blood pressure and heart rate before and after balloon occlusion and open, duration of surgeries, intraoperative hemorrhage, postoperative hip Matta score and 6 months hip functional Harris score after surgeries.Methods:Study included 43 patients of fresh acetabular fracture with Judet-Letournel type Ⅵ-Ⅹ. Study group Include 18 cases, application the temporary lower abdominal aortic balloon occlusion technology in open reduction and internal fixation of complex fresh acetabular fractures to control hemorrhage. Before open reduction and internal fixation of fracture all patients were facing aortic angiography to know the shap of abdominal aortics. renal arteries and common iliac arteries. Put in balloon catheter via the femoral artery and make sure the balloon is located in the lower part of abdominal aorta. Inject liquid in balloon to block abdominal aorta for reducing hemorrhage during the operation. Control group Include 25 previous, does not use special methods to control hemorrhage during surgery. Collection patient gender, age. type of fracture, surgical approach, hemoglobin concentration and albumin concentrations before operation, duration of surgery, intraoperative bleeding, blood pressure and heart rate before and after balloon occlusion and open, allogeneic blood transfusion volume, postoperative hip Matta score and 6 months hip functional score after surgeries by using medical records, preoperative imaging results. operation records, records of anesthesia and intraoperative nursing records. Compare the two groups by analysis the data above.Results:All patients access to more than 6 months follow-up. Analysis the data above, it’s no significant difference (P>0.05) between two groups about sex. age. type of fracture, surgical approach, hemoglobin concentration and albumin concentrations before operation, post-operative Matta score and post-operative Harris score. But the duration of surgery, intraoperative bleeding and blood product usage are difference (P<0.05).Conclusion:Application temporary lower abdominal aortic balloon occlusion during surgical treatment of complex acetabular fracture, can reduce bleeding effectively, keep the field clearly and shorten the operating time. It is a recommendable technique.
