To Build Up The Scoring Scale For Therapeutic Evaluation Of The Chinese Medicine Based On Differentiation Of Symptoms And Signs Of Liver And Spleen In The Treatment Of Diarrhea-predominat Pattern Of Irritable Bowel Syndrome

ObjectiveIn the pathogenesis of Diarrhea-predominant pattern of Irritable Bowel Syndrom, the core status of the differentiation of symptoms and signs of liver and spleen is gradually being corfirmed. But it seems to be short of adaptable kind of scoring scale for therapeutic evaluation of the Chinese Medicine treating the IBS-D of the type called liver-spleen disharmony. We aim at woring out a syndrome scoring table that could be solve the problem.MethodsFollowing the principle of the scale of both in China and iternational, in combination with related lierature, we sort out all of the syndromes of Diarrhea-predominant pattern of Irritable Bowel Syndrom(liver-spleen disharmony) to form the item pool on the basement of the study of the finishing related reseach. Frstly, we use Delphi Method to screen items to form the preliminary scale of curative effect evaluation. Secondly, we continue to use Delphi Method and Satty’s formula to quantify the items.At the same time, we use the preliminary scale to conduct the clinical investigation, aiming at counting up the frequency of clinical symptoms. At last, we combine the quantitive and qualitative data to build up the scoring scale for therapeutic evaluation of the Chinese Medicine treating the IBS-D of the type called liver-spleen disharmony.ResultsThe scale that we worked out contains two dimensions. They are dimension of disease and dimension of syndrome. The former one includes two first order items that are abdonminal pain and diarrhea. Abdonminal pain includes four second items that are frequency, duration, severity, and affetcting the life. Diarrhea includes two second items that are frequency and stool character. Dimension of syndrome includes diarrhea after abdonminal pain、 abdominal distension emotional disorders、bloating afther the meal、discomfort in chest and hypochondrium、borborygmus、flatus. Abdominal distension includes four second items that are frequency, duration, severity, and affetcting the life. The other dimension of syndromes don’t have any gradings. In addition, tongue picture and pulse are qualitative item kept in the scale.ConclusionWe had preliminarily worked out scoring scale for therapeutic evaluation of the Chinese Medicine treating the IBS-D of the type called liver-spleen disharmony. The scale will provide the foundation for later research and direction. But we need to move forward to inspection reliability validity.
