ObjectiveChronic Obstructive Pulmonary Disease(COPD) is the severe disease harmful to human health in 21st century, which making COPD the fourth course of death in global world, and will become the fifth world economic burden until 2020. Due to the smoke, environment pollution, occupational exposure and other risk factors, COPD has take the first part in disease burden sequence, which means incidence of COPD in China, a developing country, is not optimistic. According to traditional Chinese medicine, COPD is related to cough, gasp syndrome, lung distension and some other disease on the basis of syndrome differentiation. Most of the exports and scholars thinks the pathogenesis of A is related to lung, spleen, kidney and other organs, they has made great progress in research focused on pathogenesis and prevention of COPD through lung and spleen, especially through backing up earth to generate metal.The study by comparing different syndrome types of COPD patients in acute phase and stable phase with the changes in HDACs activity and cytokine levels, and expounds the relationship between HDACs and inflammatory response indirectly. And confirming the clinical effect by comparing before and after treatment in lung-spleen deficiency type with Jianpiyifei granule. In order to provide a basis that HDACs is the key of endogenous anti-inflammatory system that can treat COPD, and backing up earth to generate metal can prevent COPD through the system.MethodsSelect 37 cases of COPD patients included in the standard, and compare HDACs activity and inflammatory cytokines level between the acute phase (2nd day in hospital) and the stable phase (7+3th day in hostipal).Divided the patients into three groups with 14 lung qi deficiency group,13 lung-spleen qi deficiency and 10 lung-kidney qi of deficiency, that are in accordance with the guidelines of TCM diagnostic criteria, and observed the HDACs activity and the level of IL-6, IL-8 between the three groups. Give the lung-spleen qi deficiency group patients three months treatment with reinforcing earth to generate metal (Jianpiyifei granule combined with western symptomatic treatment), then observed the clinical symptoms, the changes of HDACs activity and IL-6, IL-8 level before and after treatment.Results(1)HDACs activity significantly in the acute stage after treatment is higher than before treatment. IL-6, IL-8 is lower than the before treatment. (2)HDACs activity from high to low is lung qi deficiency group, lung-kidney qi deficiency group, lung-spleen qi deficiency group, but the opposite in cytokines, and it had no significant difference in the three different syndrome type groups (P>0.05). (3)Clinical symptoms has been improved with significant difference(P<0.05), the total effective rate of treatment group is 92.3%; (2)The content of HDACs activity has very great difference(P<0.05) in the group of lung spleen deficiency group before and after treatment. (4)The changes of HDACs, IL-6 and IL-8 has difference(P<0.05) in the group of lung-spleen qi deficiency after the treatment of Jianpiyifei granule.ConclusionThe study shows that HDACs activity and IL-6, IL-8 level shows a negative correlation. Jianpiyifei granule can improve the clinical symptoms of COPD patients with lung-spleen qi deficiency type, it also can improve HDACs activity, and reduce the cytokinesIL-6, IL-8, which means Jianpiyifei granule has therapy effects on COPD patients with lung spleen deficiency type by endogenous anti-inflammatory system in which HDAC function is the key. |
