Objective:Compared to the commonly used operation (Semitendinosus rear plus semimembranosus and gracilis relaxation combined Ilizarov drawing technique) and modified operation (semitendinosus rear, semimembranosus relaxation with gracilis transposition combined with Ilizarov drawing technique) in the treatment of spastic cerebral palsy with flexion deformity, and consequently providing new ideas and methods for the treatment of spastic cerebral palsy with flexion deformity.Materials and methods:According to the diagnostic criteria and inclusion criteria, select 30 cases of knee flexion deformity in spastic cerebral palsy patients in September 1,2013 to June 31,2015 from the Orthopedics of Affiliated People’s Hospital of Fujian University of traditional Chinese medicine. Using the lottery method (randomization method) and randomly dividing them into two groups,15 cases in each group:1,the control group:using the semitendinosus rear plus semimembranosus and gracilis relaxation combined Ilizarov drawing technique for the the treatment of spastic cerebral palsy with flexion deformity.2, the experimental group:using the semitendinosus rear, semimembranosus relaxation with gracilis transposition combined with Ilizarov drawing technique for the the treatment of spastic cerebral palsy with flexion deformity. A half year follow-up after operation. The statistical analysis of observed indicators are using SPSS18.0 software to analysis and processing.Results:The preoperative symptoms, Dimeglio score and gait analysis results of two groups of patients were not statistically significant (P>0.05), and them were comparable.The symptoms and signs, Dimeglio score and gait analysis results before and after surgey of the control group of 15 patients with totally 30 knee for statistical analysis and consenquently all can get P<0.05, the difference was statistically significant. This means that the control group patients after surgey were all significantly improved, the traditional surgery on treatment of spastic cerebral palsy with flexion deformity has obvious curative effect.The symptoms and signs, Dimeglio score and gait analysis results before and after surgey of the experimental group of 15 patients with totally 30 knee for statistical analysis and consenquently all can get P<0.05,too. The difference was statistically significant. It means that the experimental group patients after surgey were all significantly improved, the improved surgery on treatment of spastic cerebral palsy with flexion deformity also has obvious curative effect. The symptoms and signs, Dimeglio score and gait analysis results after surgey of the two group of 30 patients with totally 60 knee for statistical analysis and consenquently all can get P<0.05, and it means that the experimental group patients after the surgery improved more than patients in the control group,the improved surgery is better in treatment. Postoperative complications:after the surgery, the control group of 2 patients with a total of 3 knee joints, the experimental group after the surgery of 2 patients with a total of 2 knee joints are both appeared suppurative infection in the nailhole of thigh. Symptomatic treatment after removing the external fixator, the nailhole has healed well. After surgery,the control group of 2 patients:1 patients with 2 knee joints has a less than 20 deformity recurrence, the other 1 patient with 1 knee joint stiffness and joint activity poor. After surgery,the experimental group of 30 knee also has a total of 2 patients with 2 knee joints has a less than 20 deformity recurrence and without knee joint stiffness. The two groups in the incidence of complications after surgery, detected by Fisher’s exact test does not exist significant differences.Conclusion:The clinical curative effect of modified operation (semitendinosus rear, semimembranosus relaxation with gracilis transposition combined with Ilizarov drawing technique) is better than commonly used operation (Semitendinosus rear plus semimembranosus and gracilis relaxation combined Ilizarov drawing technique), and it provides more choices for clinicians.
