Object:To study the inflence of the continuous venovenous hemodiafiltration on the mRNA of Toll like receptor 4、membrane cluster of differentiation 14、interleukin-1β、tumor necrosis factor-α and interleukin-10,and evaluate the potential value of TLR4 as indicator of clinical detection comparing with clinical parameters.Methods:24 suffering severe acute pancreatitis patients from the first people’s Hospital of Yunnan Province during 2013 to 2015 were randomly divided into normal control group and CWHDF group. The CWHDF group were received continuous venovenous hemodiafiltration, while normal control group only were received common treatment, then the expressions of TLR4, mCD14,IL-1β,TNF-α,IL-10 in serum were measured during 12 hours.And the clinical parameters,such as APACHE II(Acute Physiology and Chronic Health Evaluation),PCT (procalcitomin),Hs-CRP(High-sensitivity C-reactive protein) AMY(amylase),LIPA(lipase), were measured,Result:(1)The CWHDF treatment significantly suppressed the expressions of TLR4,mCD14, IL-1β,TNF-α.while the expression of IL-10 were improved.(2)The clinical parameter is correlated with TLR4,and TLR4 changed earlier then APACHE Ⅱ,PCT and Hs-CRPConclusion:(1)The CWHDF treatment attenuated the activation of TLR4 signaling pathway, thus reducing the pro-inflammatory cytokines induced by TLR4 signaling pathway.(2)TLR4 possibliely become one of the potential indicators of clinical detection of SAP development.
