The Application Research Of Endoscopic Dilatation Combined With Local Injection Of Triamcinolone Acetonide

Objective:To investigate the feasibility and value of benign anastomotic stenosis of the endoscopic dilatation combined with local injection of triamcinolone acetonide.Methods : 58 cases of anastomotic stenosis were randomly divided into two groups,the experimental group( group A) 28 cases, expanded by endoscopic Savigny dilator combined with local injection of triamcinolone acetonide,the control group(group B) 30 Cases,with simple expansion of treatment. Both groups were treated for1-9 times at intervals of two weeks. The patients were followed up for 1 year. The Groups were observed the improvement situation of eating obstruction.Results: A group: significant efficiency 53.6%(15/28), effectiveness 35.7%(10/28), inefficiency 10.7%(3/28), total efficiency 89.3%(25/28); Group B:significant efficiency 30.0%(9/30), effectiveness 26.7%(8/30), inefficiency 43.3%(17/30), total efficiency 56.7%(17/30). The differences between the total efficiency rate of the groups(significant efficiency+effectiveness) had statistical significance(P<0.05). All the cases had no massive bleeding, perforation, infection and other complications. Serious complications caused by hormone do not appear.Conclusions:It has a good effect on the treatment of benign anastomotic strictures that endoscopic dilation combined with local injection of triamcinolone acetonide,which prolongs the time when stenosis occurs again, reducesthe expansion and alleviate sufferingbrought by repeated expansion and improve the lifequality of patients.
