Objective:To research the effect of Yangxin2 Decoction on the mycoardial energy metabolism in rats with Chronic Heart Failure(CHF).To explore the mechanism of Yangxin2 decoction for the treatment of CHF.To provide objective basis for Yinxin2 decoction in the clinic application.Methods:45 male clean grade SD rats were divided into two groups,normal group(n=6) and model group(n=39).The model group was injected hypodermic with ISO at 1.0 mg·kg-1·d-1 for 14 consecutive days.Staring from the 15th day, The model group was injected hypodermic with ISO at 0.5 mg·kg·d-1 for 14 consecutive days.Meanwhile,the control group was established by injecting hypodermic same volume of same volume of normal saline. The survival rates were compared by using Doppler echocardiography procedures. After the model is successfully established in CHF, the survival rats were randomly divided into three groups:model group (group B), Carvedilol group (group C) and Yangxin2 decoction group (group D).Group C and group D were received gastric gavage of Carvedilol and Yangxin2 decoction respectively. The model group and group B were received gastric gavage of distilled water. After 28 days of dosing, the levels of myocardial adenosine triphosphate (ATP) and the levels of serum NO and iNOS were tested.Rat myocardial tissue pathological biopsy also were tested.Results:1 The results of the rats cardiac color ultrasound compared between groups:Compared with the control group(group A),the LVEDD,LVESD of the model group (group B) were significantly increased (P<0.05), LVEF and LVFS were decreased(P<0.01); Compared with the model group (group B), the LVEDD,LVESD of the two drug treatment groups(group C and group D) were decreased(P<0.05),LVEF and LVFS were significantly Increased(P<0.05);Compared with the group C,the LVEDD,LVESD, LVEF and LVFS of group D were no statistical difference (P> 0.05).2 The levels of myocardial ATP compared between groups:Compared with the normal group (group A), the levels of myocardial ATP was decreased in the model group(group B)(P<0.01); Compared with the model group(group B), the levels of myocardial ATP was increased in the Yangxin2 decoction group (group D) (P<0.01).Compared with the Carvedilol group (group C), the levels of myocardial ATP was increased in the Yangxin2 decoction group (group D) (P< 0.01).3 The levels of serum NO compared between groups:Compared with the normal group(group A),the levels of serum NO were increased in the model group (group B)(P<0.01).Compared with the model group (group B), the levels of serum NO were increased in the Carvedilol group (group C) (p<0.05). Compared with the model group (group B), the levels of serum NO were increased in the Yangxin2 decoction group (group D) (p<0.05). Compared with the group C, the levels of serum NO of group D was no statistical difference (P> 0.05).4 The levels of serum iNOS compared between groups:Compared with the normal group(group A),the levels of serum iNOS were increased in the model group (group B) (P<0.01).Compared with the model group (group B), the levels of serum iNOS were increased in the Carvedilol group (group C) (P<0.05). Compared with group B, the levels of serum iNOS were increased in the Yangxin2 decoction group (group D) (P<0.01). Compared with the group C, the levels of serum iNOS of group D was no statistical difference (P>0.05).5 The images of the Rat myocardial tissue pathology compared between groups:In the control group(group A), the myocardial tissue structure is clear,cardiac muscle cells are uniform and cardiac muscle fibers are orderly. Compared with the control group(group A),the myocardial nucleus and cells are wider in the modle group(group B). The myocardial cells are disordered arrangement and loose. Some parts of muscle cells are necrotic. Compared with the modle group(group B), some parts of myocardial tissue structure is more orderly,but a few regional of myocardial cells is still swollen in the Carvedilol group(group C). The inordinate order of muscle fibers is alleviative.In the Yingxin2 decaction group(group D), myocardial tissue structure is as normal,neatly arranged,with no swelling,dissolution.Conclusion:(1) The CHF model induced by injecting hypodermic ISO continuously is a simple and convenient method.(2) Yangxin2 decoction could improve cardiac function of rats with Chronic heart failure.(3) Yangxin2 decoction could increase the concentration of myocardial ATP and the level of of serum NO and iNOS, improve myocardial energy metabolism and protect myocardial tissue structure.This may be one of the functionary mechanism of Yangxin2 decoction to treat chronic heart failure.(4) The effect of increaseing the concentration of myocardial tissue ATP of Yangxin2 decoction is better than Carvedilol,but the effect of reducing the levels of serum NO and iNOS is almost the same of Yangxin2 decoction and Carvedilol.
