Objective:To evaluate the feasibility and effectiveness of prostate artery embolization (PAE) in the treatment of benign prostatic hyperplasia (BPH),To investigate the causes and treatment strategies of complications in the treatment of BPH with prostatic artery embolization (PAE).Methods:PAE operation was performed in 7 patients with BPH,1 months,6 months and 12 months after operation follow-up and the clinical results were collected,observed and compared before and after operation in patients with International Prostate Symptom Score (IPSS).Quality of Life (QOL).Prostate Volume (PV) (B ultrasound), Peak Urinary Flow (Qmax) and Residual Urine (RU). Perioperative complications and treatment outcome were observed.Results:A total of 12 prostate artery (PA) in 7 patients were treated with PAE.7 prostate artery originated from the inferior vesical artery,2 from the obturator artery, 2 from the internal pudendal artery,1 from the inferior gluteal artery. Unilateral embolization in 2 cases, bilateral embolization in 5 cases.12 months after PAE, IPSS, QOL, RU, PV(B ultrasound), from the preoperative (average) (28.0±3.6) points, (6.0±1.7) points,(132.1+75.3)ml, (106.3±30.8)ml, respectively, down to (2.1 ±0.3) points, (0.3±0.7) points, (2.9±2.1) ml, (54.0±10.9) ml,Qmax from the preoperative 6.5+2.7ml/s to 17.3+1.6ml/s.The success rate of PAE was 100%(7/7), Complications occurred in 3 cases, including 1 cases of aortic dissection,1 cases of non target vessel embolism, No permanent damage was formed after corresponding treatment.Conclusion:PAE is effective in the treatment of BPH(when it is difficult to intubate in special circumstances,unilateral embolization can also achieved a very good therapeutic effect),with high success rate and few severe complications. It can be used as a first-line treatment for BPH. A careful analysis of collateral and blood circulation of the arteries that supply blood to the prostate,superselective catheterization,fine operation,precise embolization and active prevention and treatment of complications is guarantee of clinical efficacy.
