Clinical Effect Observation Of Modified Method Of Decortication Of Pleural Fibreboard With Video-assisted Thoracoscopy For Chronic Empyema

Objective:To explore the clinical application of modified method of decortication of pleural fibreboard with video-assisted thoracoscopy for chronic empyema.Methods:In the traditional open thoracotomy and VATS based on October 2013 2015 December of Kunming Medical University the Fourth Affiliated Hospital of Department of thoracic surgery, the 31 cases of chronic empyema were treated by the modified the two hole thoracoscopic pleural fibreboard stripping off therapy, the clinical data of all patients were retrospectively analysis and to observe the effects.Results:27 cases were performed under the modified VATS successfully,4 cases were transferred to video-assisted muscle-sparing thoracotomy. The operation time was 120 to 185 min, the average was (145±42) min, and the average blood loss was (380±100) ml. The time of the thoracic drainage tube was 4-16 days after operation. There were one case occurred a persistent lung leakage more than 7 days and one case occurred a pleural effusion more than 7 days. The average length of postoperative hospital stay was 7-14d. All patients were followed up for 3 to 6 months, and 1 patient was found with middle amount of pleural effusion, and the rest of these patients were found without empyema recurrence.Conclusion:The modified method of decortication of pleural fibreboard with two-hole video-assisted thoracoscopy had the advantages of both conventional thoracotomy and routine VATS, and expanded the surgical indication of VATS for chronic empyema.
