HTP Immediate Perfusion And Postoperative Week Filling To Prevent The NMIBC Recurrence After Curative Effect Comparison

Objective:This paper mainly aims to provide practical and reliable theoretical foundation for further clinical treatment after undergoing surgery of NMIBC.For patients diagnosed with NMIBC is divided into two groups, separately THP immediate perfusion and postoperative week perfusion,observe different perfusion after resection of the tumor recurrence rate and adverse reactions.Methods:104 patients selected from Qinghai University Affiliated Hospital from Jan. 2011 to Jan. 2015 as research objects and recorded patients’ gender, age, whether starting, tumor size, number, grade, stage, etc.The lines operation method for all patients with TURBt,postoperative pathological diagnosis of NMBC, were retrospectively analyzed.Cases were divided into two groups.One group is the 54 patients of NMIBC confirmed in Qinghai University Affiliated Hospital from Jan. 2011 to Jan. 2013 were irrigated THP in bladder for the first time after one week of undergoing TURBt,continuous perfusion after 1 year,which were marked as irrigation group after one week of surgery; Another group is the 50 patients of NMIBC confirmed in Qinghai University Affiliated Hospital from Jan. 2013 to Jan. 2015 were irrigated THP in bladder for the first time immediately when the surgery had been finished, continuous perfusion after 1 year,which were marked as group of immediate irrigation. The recurrence rate and untoward effect of carcinoma of urinary bladder of the patients after undergoing TURBt by different irrigation method had been compared.Results: The recurrence rate of patient of carcinoma of urinary bladder in irrigation group after one week of surgery was 38.9%, while the rate in group of immediate irrigation was only 18.0%, hence the difference between the them was with statistical significance(P<0.05), which means that the occurrence rate of carcinoma of urinary bladder in group of immediate irrigation was lower than that of carcinoma of urinary bladder in irrigation group after one week of surgery; the differences of adverse reaction, such as irritation symptoms of bladder, fever, swelling at urethral orifice and nausea and vomiting, between the two group were without statistical significance(P>0.05); the occurrence rate of gross hematuria in irrigation group after one week was 5.6%, while the rate of gross hematuria in group of immediate irrigation was 24.0%, hence the difference between the them was with statistical significance(P<0.05), which means that the gross hematuria in group of immediate irrigation was easier to get than that in irrigation group after one week of surgery; the gender and or-not first-episode of patient had no statistical significance(P>0.05) upon occurrence rate of the carcinoma; the age of patients,neoplasm staging,neoplasm grading and tumorous numbers had statistical significance(P<0.05) upon occurrence rate of the carcinoma.Conclusions:1. The therapeutic effect upon preventing occurrence of the carcinoma for patient of NMIBC after undergoing TURBt who was irrigated THP for the first time immediately was better than that of patient who was irrigated THP after one week of the surgery; 2. There was no significant difference upon adverse reaction, such as irritation symptoms of bladder, fever, swelling at urethral orifice and nausea and vomiting, after undergoing surgery of TURBt to cure NMIBC; the occurrence rate of gross hematuria in group of immediate irrigation was higher than that in irrigation group after one week of surgery; 3. The higher of the neoplasm staging and grading of patient of NMIBC, the higher of occurrence rate of the carcinoma was, the higher of age of patients, the higher of occurrence rate of the carcinoma was,and the larger number of carcinoma, the easier of occurrence of the carcinoma was; 4. The long-term therapeutic effect of preventing occurrence rate of patient of NMIBC after undergoing TURBt by irrigating THP immediately should be studied further.
