Objective:To investigate the 11β-HSD1, AMPK, GLUT-4 levels of peripheral blood lymp hocytes in patients of diabetes mellitus type 2 before and after oral metformin.Methods:48 cases, who were outpatients, or for physical examination in the First Affilia ted Hospital of Shanxi Medical University from January 2015 to September 2015,were involved in this research according to the inclusion criteria and they were cat egorized into 2 groups: 24 cases of normal control group(NC group), 24 cases of type 2 diabetes mellitus(T2DM group), 7 cases were collected after treated by mer formin. The data of the cases such as age, gender, history of present illness, past medical history were recorded. The levels of 11β-HSD1, AMPK, GLUT-4 of periph eral blood lymphocytes for each group were measured by qPCR, the HbA1 c was al so tested. All data were analyzed by t-test, to test 11β-HSD1, AMPK, GLUT-4 leve l in patients of diabetes mellitus type 2 before and after oral metformin.Results:With GAPDH as an internal, newly diagnosed type 2 diabetes patients and hea lthy compared to peripheral blood lymphocytes on AMPK, GLUT-4 expression was no significant difference, 11β-HSD1 expression 94.04% lower than the healthy peopl e. After 16 weeks on metformin, 11β-HSD1 increased 6.82 times compared with th e former, GLUT-4 expression increased 51.72% than before, the difference was statistically significant. AMPK relatively little change before taking. Correlation analysis showed: AMPK with a correlation coefficient of 11β-HSD1 was 0.347(P <0.05),AMPK and the correlation coefficient GLUT-4 was 0.322(P <0.05), 11β-HSD1 wit h a correlation coefficient of GLUT-4 was 0.292(P <0.05), the correlation coefficie nt 11β-HSD1 and HbA1 c was-0.302(P <0.05). AMPK and HbA1 c of P >0.05, G LUT-4 with HbA1 c of P >0.05.Conclusion:(1) newly diagnosed type 2 diabetes in patients with 11β-HSD1 expression in l ymphocytes is low;(2) after taking metformin on peripheral blood lymphocytes was significantly increased 11β-HSD1, which may be by increasing peripheral blood ly mphocytes expression of glucocorticoid to enhance immune function, thereby strengt hening the anti-inflammatory effect;(3) after taking metformin on peripheral blood l ymphocytes did not change significantly compared with the previous expression of AMPK, probably because of the less sample volume, is not sufficient to reflect the differences;(4) patients taking metformin after GLUT-4 expression in peripheral bl ood lymphocytes increased, 11β-HSD1 may be used to promote glucose transport in to cells by activating the expression of GLUT-4, thereby strengthening the lymphoc yte metabolism to promote their physical functions, including immune protection.
