Double Ends Compressive Screw Dorsal Percutaneous Minimally Invasive Fixation For The Treatment Of Scaphoid Bone Fractures

ObjectiveTo investigate the surgical methods and clinical efficacy of double ends compressive screw dorsal percutaneous minimally invasive fixation for the treatment of scaphoid bone fractures. MethodsFrom December 2013 to November 2015,19 patients with fresh wrist scaphoid fracture in my department, they were treated by double ends compressive screw dorsal percutaneous minimally invasive fixation. The results of the procedure were assessed in terms of fracture healing time, active motion of the wrist, pain extent, return to work time and grip strength. Postoperative outcomes were evaluated by wrist standard scores based on patients’ perception of functions. ResultsPostoperative were primary healing of incision, no incision infection、no healing of incision ect postoperative complications,all the 19 cases were followed up for 6-12 months, with an average followed up for 9 months, the fracture healing time was(12±2.94)weeks. The last follow-up of 19 patients with postoperative VAS score was(1.00±0.882)points, Significantly lower than the preoperative VAS score(5.89±1.595)points(t=11.707,P=0.000); Postoperative grip strength was(35.895±6.665)Kg, which was significantly higher than that preoperatively(19.553±3.726)Kg(t=-9.329,P=0.000);Limb function of wrist joint activities: the angle of preoperative motion flexion and extension was(82.132±7.646)°, Postoperative was(108.426±7.838)°(t=-10.467,P=0.000), Preoperative ulnar radial deviation degree was(39.737±3.143)°,Postoperative was(55.211±4.101)°(t=-13.044,P=0.000),It can be obviously seen that postoperative limb wrist activity was more improved than preoperative. According to Cooney wrist function evaluation: 17 case were excellent, 1 case was good, 1 case was fair, the excellent and good rate was 94.7%. Postoperative return to work time was(14.2±2.54)weeks. ConclusionsDouble ends compressive screw dorsal percutaneous minimally invasive treatment with fresh wrist scaphoid fracture is a kind of way to treat wrist scaphoid fracture, which has the advantages of simple, small trauma, reliable curative effect, and easy to be accepted by patients, postoperative limb joint functional exercise early, reduced nonunion complications and so on, double ends compressive screw dorsal percutaneous minimally invasive treatment with fresh wrist scaphoid fracture curative effect is satisfied.
