| Objective: Through the incidence survey on the Chengguan District of Lanzhou city community elderly primary osteoporosis(POP). Analysis and comparison of three diagnostic criteria of osteoporosis in the elderly population in Lanzhou City, a preliminary study on the influence factors of primary osteoporosis and its relationship with the type of TCM constitution. In order to provide scientific basis for the local district and the district administrative departments in the prevention and treatment of osteoporosis this public health problem.Methods: Take the stratified cluster random sampling method, Chengguan District over the age of 50 community staff of 2500 people. For primary osteoporosis influence factors and TCM Constitution questionnaire survey, collected survey of blood uric acid(UA), blood lipid(TC,TG,HDL- C,LDL- C) content. The measurement survey of body mass index(BMI) and lumbar(L2, L3, L4) and hip(Neck, Ward’s triangle, G.T.) bone mineral content(BMD) and T value. The judgment standard for TCM Constitution identification by TCM Constitution in 2009 issued by the China Association of Chinese Medicine. Used exercise prescription and nutrition diet formula to obtain the status of the investigation of outdoor sports and the status of the daily calcium intake. The data processing used SPSS 17.0 software, applied to the WHO(SD) standards, OCCGS(SD) standards and OCCGS(%) standards, compared the detection rate of POP and the consistency of the diagnosis, and analysis correlation with TCM constitution type of POP.Results: The WHO(SD) standards, OCCGS(SD) standards and OCCGS(%) standards POP detection rate are of 13.89%, 25.65% and 45.32%. Respectively, WHO(SD) standards and OCCGS(SD) standards, WHO(SD) standards and OCCGS(%) standards and OCCGS(SD) standards and OCCGS(%) standards POP consistent detection rates are 13.89%, 13.69% and 25.44%.The WHO(SD) standards and OCCGS(SD) standards were highly consistent with the OCCGS(SD) standards and OCCGS(%) standards is moderate, WHO(SD) standards and OCCGS(%) standards showed low consistency.Age and bone mineral density(BMD) are negatively correlated(P<0.05);there is no difference between the sexes of BMD(P>0.05);BMI,UA,E2,T,TG,TC, outdoor exercise and diet(calcium intake) is the influencing factors of BMD, and this is a positive correlation;HDL-C and LDL-C had no correlation with osteoporosis; Logistc analysis showed that the serum lipid is confounding factors are excluded.1957 people in the normal bone mass detection rate is 39.3%(770 people). The TCM constitution has differences in sex distribution(P<0.05). There is statistical significance level and quality and the damp heat, the remaining seven kinds of physical differences in bone mass distribution in the normal population.1957 people, 502 people suffer from osteopenia, the detection rate is 35%. Age as a risk factor for osteopenia; Blood stasis, Qi stagnation, Yin deficiency,Yang deficiency,Phlegm dampness,Qi deficiency 6 biased constitution is a risk factor for osteopenia,Which Qi deficiency occur the risk of osteopenia is 10.2 times the level and quality, Yang deficiency constitution bone mass and decrease the risk degree is 13.1 times the level and quality, the risk of osteopenia in Yin deficiency is 13.5 times the level and quality, Phlegm dampness constitution bone mass and decrease the risk degree is 11.5 times normal quality and the risk of osteopenia in Blood stasis is 18.9 times of that of normal quality, stagnation of Qi, the risk of osteopenia is 18.9 times of that of normal quality.1957 people, 502 people suffer from POP, the detection rate of 25.7%.Results showed that 6 kinds of physical variables P<0.05, with statistical significance, and all are risk factors for osteoporosis. According to the order of OR value, the descending order of Qi stagnation>Yang deficiency>Blood stasis>Qi deficiency>Yin deficiency>Phlegm dampness.Conclusion: WHO(SD) standards is not suitable for lanzhou chengguan district elderly people, OCCGS standards criteria is recommended for the population in Lanzhou City, OCCGS(SD) standards can be applied in clinical diagnosis, OCCGS(%) standards is more suitable for epidemiological investigation.Chengguan District of Lanzhou city in the elderly primary osteoporosis rate is 25.65%.And Shenyang, Wuhan, Xiamen, Shenzhen and other places a number of research results compared to slightly lower. It should be appropriate to increase the amount of exercise and outdoor exercise time, appropriate to increase the consumption of calcium and purine-rich foods, maintaining appropriate body mass index for Chengguan District of Lanzhou city people in daily life. Physical type Lanzhou City in bone mass in the normal population is Gentleness and Humid quality. The survey found that elevated levels of estrogen humid mass crowd, preliminary think that Damp heat is one of the protective physique of bone mineral density, and the specific mechanism needs further study. Osteopenia and osteoporosis of the people, the same type of constitution is Qi deficiency,Yang deficiency,yin deficiency, phlegm dampness, blood stasis, qi stagnation of the 6 kinds of biased constitution. The elderly of the community in the prevention and treatment of osteoporosis recommends applying "Chinese physical identification". Qi deficiency, Yang deficiency, yin deficiency, phlegm dampness, blood stasis, qi stagnation, such as 6 kinds of biased constitution of the population to reduce the population as the focus of prevention and control of the object. |
