| Objective:Evaluation the hemodynamic effects and postoperative pharyngeal complications of Macincosh direct laryngoscope, videolaryngoscope and laryngeal mask(number 4 or 5) under the same anesthesia depth(BIS=40).Methods:80 patients(aged 20 to 60 years old, weight 55-75 kg, ASA phsical status I-II), scheduled for elective not laparoscopic surgery under general anesthesia, were randomly divided into the following 4 groups(n=20): Macincosh direct laryngoscope(DL), video laryngoscope(VL), laryngeal mask of model number 4(F4) and 5(F5). To record SBP, DBP, HR and calculate HR×SBP value on the time before anesthesia(T0), after induction(T1), laryngeal mask placement or the immediate time of intubation(T2), 3 minu after intubation(T3) and 5 min after intubation(T4).Results:1 There were no statistically significant difference among the four groups of patients’ age, height, ASA class, and operation time(P>0.05).2 For the four groups, compaired with the moment of T0,SBP, DBP, SBP×HR and HR of the moment of T3 were significantly decreased by the induction of anesthesia, but transiently incresased in the moment of T2 by intubation(P<0.05). In the moment of T3, SBP, DBP, HR×SBP were comging back to the baseline level. There were no significant differences of changes of hemodynamic between the time T3 and T4(P<0.05). At the moment of T2, DL group has bigger influence on the SBP, DBP, HR×SBP and HR than VL, F4 and F5(P<0.05). However, there were no significant differences among VL, F4 and F5 group(P>0.05). For the effects of the four groups made on HR, DL was the biggest, the second was VL group, F4 and F5 were the minimum(P<0.05).3 The comparisions of 24 hour postoperative pharyngeal complications of the four groups are DL group was the highest, VL group was higher, F4 and F5 were the minimum. And for the 48 hours, there were no differences among the four groups(P>0.05).Conclusion:video laryngoscope, laryngeal mask have less influence on hemodynamics than direct laryngoscopy, and also have low postoperative pharyngeal complication incidence rate, which are suitable for clinical application. |
