| Objevtive To research the prevalence and treatment status of hypertension among the middle-aged and elderly residents in Gansu Province, and obtain the latest prevalence of hypertension and its basic characteristics as well as independent risk factors of the middle-aged and elderly residents in Gansu Province. The latest treatment status of hypertension in Gansu Province could be found as a theoretical foundation for the government to make the further integrated control measure of the middle-aged and elderly population.Method According to multi-stage cluster random sampling principles, a total of 20160 sample families were randomly chosed from the urban and rural area of the province-wide, and aged ≥45 years residents were chosen to be respondents. With the method of telephone appointment, concentrating survey and entering home to accomplish the questionnaire survey and physical examination. More than two person inputed the survey data with the software EpiDate3.1twice, and all data were processed by SPSS21.0 software to obtain the statistical analysis data of the prevalence and treatment status of this survey. And then respondents were grouped according to different demographic characteristics to compare the differences between each other. And correlation analysis was made to determine the risk factors of the middle-aged and elderly hypertensive in Gansu Province.Result ①The prevalence of middle-aged and elderly hypertensive in Gansu Province was36.20%, the standardized prevalence rate was 35.09%, of which prevalence rate was 35.23% in male(standard rate was 33.92%), and it was 37.10% in female(standard rate was 36.05%). ②The prehypertension detection rate was 39.74%, the awareness rate of hypertension was 50.47%,treatment rate was 40.15%, and the control rate was 17.56%, which was significantly lower in rural than urban areas(all P <0.01). Moreover the prevalence rates of hypertension in urban and rural were 35.96% and 36.44%, respectively. ③ According to the results of this survey, the hypertension prevalence rates of middle-aged and elderly residents were 37.31%, 36.94% and35.82% in Hui, Tibetan, Han and the prevalence of Hui, Tibetan population was significantly higher than the Han population(P<0.01). Furthermore there was no significant difference between the different genders in Hui and Tibetan about the hypertension prevalence(P<0.01). ④ The analysis results showed that low levels of education, low per capita income, non-working,smoking, drinking, over salt diet, high-fat diet, uncontrol diet, family history, inactivity,overweight, obesity, central obesity, diabetes, high triglycerides and other factors were the major risk factors of middle-aged and elderly hypertensive in Gansu Province.⑤ As far as the whole Province concerned, the regional differences of prevalence were significantly great. ⑥The rate of stroke in the middle-aged and elderly residents was 6.0‰ in Gansu Province. ⑦ There was a big difference between the urban and rural in the diagnosis way of hypertension. In the urban areas,42.55% of the middle-aged and elderly population due to the symptoms of hypertension saw the doctor. Meanwhile 42.81% of the hypertensive in rural was discovered due to the treatment of other diseases. ⑧ As the results showed that lifestyle intervention significantly promote the standard blood pressure in hypertensive patients(P<0.01). ⑨ The combination therapy rate of hypertensive population in GanSu Province was23.73%, which accounted for 83.91% of traditional compound preparation, the proportion of hypertensive with good medication compliance was only 53.56%.Conclusion Hypertension prevalence of middle-aged and elderly residents in Gansu Province was significantly higher than the 10 years ago, and hypertension awareness, treatment and control rates remain at a low level. There were a number of risk factors for hypertension prevalence in GanSu, and the blood pressure was affected by many unhealthy lifestyles. Meanwhile there were many unreasonable choose of the antihypertensive drug. And the poor medication compliance,poor blood pressure control was widely existed in Gansu province. |
