The Value Of Anti-mutated Citrullinated Vimentin Antibodies And Some Associated Antibodies In Juvenile Idiopathic Arthritis And Rheumatoid Arthritis

Objective:To compare the diagnostic value of anti-mutated citrullinated vimentin antibodies and some associated antibodies in juvenile idiopathic arthritis and rheumatoid arthritis.To analysis for the relation between antibodies and inflammatory markers.To study the association among smoking,some associated antibodies,disease activity in rheumatoid arthritis.Methods:A retrospective analysis method. Antibodies to cyclic citrullinated peptides and anti-MCV antibodies were detected by enzyme-linked immunosorbent assay(ELISA),antiperinuclear factor(APF) and antikeratin antibody(AKA) by indirect immunofluorescent assay,as well as RF by latex agglutination test, in serum samples from 113 patients with JIA,632 patients with RA, 102 adult without rheumatoid arthritis and 56 children without rheumatoid arthritis.Results:1.The area under the ROC curve of anti-MCV antibodies, anti-CCP antibodies,RF was0.922、0.935、0.846(P <0.01).2.Anti-MCV antibodies level was positively related to DAS28,ESR,CRP, tender joint counts. RF level was positively related to DAS28,ESR,CRP. Anti-CCP antibodies was not correlation to disease activity.3.The smoker positive rate of anti-MCV antibodies, anti-CCP antibodies and RF were1.631 times, 1.489 times and 1.350 times to non-smoker. Their 95%CI were(0.721,3.690),(0.573,3.871),(0.781,2.334).4.The CRP level of smoker(sx ?=38.20±64.30) were higher than non-smoker( sx ? =29.80±51.25)(t=-2.653, P < 0.05). There was not related to the other disease activity. The level of antibodies the other disease activity were not changed with smoking quantity. The CRP level of patients who smoked 1 to 9 pack-years was lower than the 10 to 19 pack-years and ≥20 pack-years patients(P<0.05).5.The positive rate of anti-MCV antibodies, anti-CCP antibodies, RF was 16.8%, 14.2%,21.2% in the JIA. In the other group, the positive rate was 2.2%,2.2%,6.5%. There was a difference between the two group(χ2 = 8.105、6.337、7.036,P <0.05). The positive rate of AKA,APF was no significant difference. The area under the ROC curve of anti-MCV antibodies, anti-CCP antibodies, RF, AKA,APF was 0.579,0.561,0.578,0.539,0.505. 6. In the RF-positive polyarticular patients, they were higher than other subtypes(P<0.05).Antibodies were no significant difference(P > 0.05) of other subtypes. 7.The swollen joint counts and tender joint counts were low correlation to anti-MCV antibodies,anti-CCP antibodies,RF,AKA,APF. No correlation between ESR, CRP with anti-MCV antibodies,anti-CCP antibodies,RF, AKA,APF.Conclusion:The diagnostic value of anti-MCV antibodies and anti-CCP antibodies is high.Anti-MCV antibodies and RF were better than anti-CCP antibodies in reflecting disease activity. It was not considered that smoking was a risk of anti-MCV antibodies, anti-CCP antibodies, RF. Smoking has little effect on disease activity. Anti-MCV antibodies have diagnostic value of RF positive polyarticular. Antibodies were related to the swollen joint counts, tender joint counts in JIA.
