| Objective:The post-operative relapse rate of patients with border ovarian tumors(BOT) and the diagnosis and treatment of relapse tumors have become increasingly important. For there are more controversial in clinical, in this study, through understanding the clinical and pathological features of BOT and post-operative recurrence in patients with BOT, we explore the effect of characteristics of the tumor and treatment on prognosis of BOT.Thus, it can provide new plots for the future diagnosis and treatment of BOT.Methods:We collected 123 borderline ovarian tumors patients which had received relative treatment and confirmed by postoperative pathological examination as the research object in the 4 tertiary hospitals in our city(the first hospital, the third hospital, the five hospital and the general hospital with the Datong coal group) from January 2009 to December 2014. The information of general demographic characteristics, clinical pathological characteristics and the influence factors of recurrence of BOT have been collected through medical record data. The general situation of research object and tumor pathological classification and so on, have been carried on description analysis. And through the case-control study, we analyze the influence factors of post-operative recurrence of BOT.Results:(1) In this study, 123 ovarian tumors patients with post-operative recurrence rate was 20.32%, has obvious age-related. Recurrence group average age was significantly lower than that of recurrence group, and the difference was statistically significant(t=3.872, P<0.001). Recurrent group patient’s age more concentrated between the age of15 to 35 years old.(2) Serous BOT(56.9%) is primary pathological type of BOT patients, followed by mucus sex BOT(35.8%). The analysis showed that there was no statistically significant difference of the recurrence rate between different tumor types(P=0.708).(3) We only found patients with Clinical stage I and II BOT, in which Ia and Ib is primary. By the analysis of different clinical stage BOT post-operative recurrence rate is different, and has statistically significant difference(P<0.001). Further do trend chi-square test according to the clinical stage, we found that the more serious with the clinical stages, the more increasing the risk of post-operative recurrence of BOT(trend2x =41.127, P<41.127).(4) After conducted single factor analysis, we found that there are five indexes different between recurrence group and non-recurrence group, including tumor size,clinical stage, micro nipple growth status, interstitial infiltrates, and peritoneal planting situation, and was statistically significant; In multiariable logistic regression analysis of BOT post-operative recurrence, there are age and clinical staging two variables into the regression equation. That is to say, under the condition of the other factors unchanged,compared with patients aged older than 55 years, the risk of recurrence increase 28.59 time in patients with aged less than 25 years and increase 12.75 time in patient aged between 25 to 35 years old respectively; Compared with the stage Ia, the recurrence possibility of clinical stage Ib, Ic and stage II was increased 12.39, 58.83 and 188.16 times respectively.Conclusion:The pathological type of borderline ovarian tumors patients is mainly serous BOT,followed by mucus sex BOT. Border oarian tumor’s prognosis is well on the whole, but it has obvious age-related; At the same condition, the smaller the age the recurrence rate is higher. Post-operative recurrence of the disease is also associate with the tumor size, the growth form, scope and clinical stage. The higher the level of clinical stage, the higher the postoperative recurrence rate. |
