Clinical Characteristics、Diagnosis And Treatment Of Secondary Epilepsy In Children

Objective To discuss the etiology、age at onset、clinical characteristics、treatment and outcome of secondary epilepsy in children.Methods Clinical characteristics、diagnosis and treatment in cohort of 96 children with secondary epilepsy treated in our department from January 2013 to May 2015 were retrospectively reviewed.Results(1)Encephalitis was a common cause of secondary epilepsy in children.(2)Post-traumatic epilepsy(PTE)was more common in infants and young children(P<0.05).( 3) The seizure types of secondary epilepsy were mainly focal with secondary generalization among groups(41%).(4)The proportion of patients that applying monotherapy was higher than patients that applying polytherapy(χ2=10.801,P=0.013).The rate of patients that applying monotherapy in PTE group(85%)was higher than patients in encephalitis secondary epilepsy group(41.7%)(χ2=10.687,P=0.001).(5)Most patients were given priority to topiramate(TPM)、levetiracetam(LEV)、valproate(VPA)applied alone or in combination.There was no statistically significant difference among groups that applying TPM alone or in combination(P>0.05).Cerebral dysplasia secondary epilepsy group had the highest percentage of applying LEV alone or in combination than other groups(χ2=8.889,8.691,7.479,P<0.01),and encephalitis secondary epilepsy group was VPA(χ2=7.083,8.333,P<0.01).(6)The outcome of children among groups had statistically significant difference(Hc=9.345,P=0.025). The outcome of PTE group was better than encephalitis secondary epilepsy group(Z=-2.778,P=0.005).There was no statistically significant difference among other groups(P>0.05)after comparing between every two groups.Conclusions 1、Encephalitis was a common cause of secondary epilepsy in children.2、PTE was more common in infants and young children.3、The seizure types of secondary epilepsy were mainly focal with secondary generalization.4、The main therapy method of secondary epilepsy was monotherapy,the rate of applying monotherapy in PTE group was especially higher than other groups.5、Good curative effect can be obtained by applying LEV alone or in combination in cerebral dysplasia secondary epilepsy group, and VPA in encephalitis secondary epilepsy group.6 、 The curative effect of PTE group was the best,while the encephalitis secondary epilepsy group was the worst.
