Objective: Gastric cancer is a common malignant tumor in our country, which seriously threaten the health of human beings.And its incidence rate and mortality rate are high. Invasion and metastasis are the main factors that affect the prognosis and quality of life of patients with gastric cancer. The process of invasion and metastasis of gastric cancer is very complicated, and it is regulated by many genes, including proteolytic enzymes and metastasis inhibitors. In recent years, with the in-depth study of tumor related factors, the mechanism of invasion and metastasis of gastric cancer has a further understanding, which also plays a guiding role in the prognosis of patients. The factors related to the invasion and metastasis of gastric cancer are searched for, and explored the changes and roles in the development of gastric cancer.it is also helpful to the early diagnosis of gastric cancer, and provide theoretical basis for targeted therapy.In this study, 140 cases of gastric cancer patients with tumor tissue were selected and clinical and prognostic data of patients were collected in order to investigate the relationship among the protein expression of Vav1,MMP-2 and TIMP-1 clinical biological behavior, and to explore the relationship among them.Methods: The specimens of 140 cases with gastric cancer were collected, which were received surgery and diagnosed finally with gastric cancer from January,2009 to July, 2010, in the fourth hospital of Hebei Medical University. All the specimens of gastric cancer and normal tissue were immunohistochemistry dyed for investigating the relationships among the expression of them and clinical biological behavior of the patients,and analyzing the correlation among their expression.Results:1 Expression of Vav1 in gastric cancer and its relationship with clinical biological behavior:The expression of Vav1 in gastric cancer were higher than the normal tissue(P<0.001). The expression of Vav1 in gastric cancer were correlated with the size,degree of tumor differentiation, tumor infiltration depth,lymph node metastasis and TNM stage(P=0.001,0.005, <0.001,0.019.0.013),but not related with sex, age, site, gross type,vascular tumor thrombus and distant metastasis(P=0.378,0.534,0.460,0.239,0.977,0.568). The 5 year survival rate of Vav1 positive patients was 50%, and the negative was 76.92%, P=0.002.There were significant differences.Prognostic analysis indicated that the positive expression of Vav1 was an independent risk factor affecting the prognosis of patients, and the risk of death was 2.139 times higher than that of patients with Vav1 negative expression(P=0.022). Vascular tumor thrombus was also an independent risk factors,the patients in the risk of death with positive was 2.830 times higher than negative patients(P = 0.001) and TNM staging was an independent risk factors for the prognosis of patients, the stage III-IV patients risk of death was 261.5 times higher than I- II patients(P = 0.002).2 Expression of MMP-2 in gastric cancer and its relationship with clinical biological behavior:The expression of MMP-2 in gastric cancer were higher than the normal tissue(P<0.001). The expression of MMP-2 in gastric cancer were correlated with the size,depth of tumor invasion, lymph node metastasis and TNM stage(P=0.039,<0.001,0.016, <0.001), but not related with sex, age, site, gross type,degree of tumor differentiation,vascular tumor thrombus and distant metastasis(P=0.915,0.072,0.080,0.462,0.125, 0.353,0.198). The 5 year survival rate of MMP-2 positive patients was 51.89%, and the negative was 85.29%, P=0.001.There were significant differences.Prognostic analysis indicated that the positive expression of MMP-2 was an independent risk factor affecting the prognosis of patients, and the risk of death was 2.734 times higher than that of patients with MMP-2 negative expression(P=0.038). Vascular tumor thrombus was an independent risk factor affecting the prognosis of patients, and the risk of death was 2.720 times higher than that of patients with negative expression(P = 0.002). TNM stage was also an independent risk factors for the prognosis of patients, the risk of death in patients with stage III-IV was 2.394 times higher than I-II patients(P = 0.006).3 Expression of TIMP-1 in gastric cancer and its relationship with clinical biological behavior:The expression of TIMP-1 in gastric cancer were lower than the normal tissue(P<0.001). The expression of TIMP-1 in gastric cancer were correlated with tumor invasion depth(P=0.015). But not related with sex, age,site,size, gross type,tumor differentiation, vascular tumor thrombus,lymph node metastasis, distant metastasis and TNM stage(P = 0.607,0.958,0.317,0.139,0.267,0.560,0.348,0.795,0.960,0.239). The 5 year survival rate of TIMP-1 positive patients was 75.86%, and the negative was 48.78%, P=0.001.There were significant differences.Prognostic analysis indicated that the negative expression of TIMP-1 was an independent risk factor affecting the prognosis of patients, and the risk of death was 0.31 times higher than that of patients with TIMP-1 positive expression(P<0.001). Tumor invasion depth was an independent risk factors for the prognosis of patients, the risk of death in the patients with T3-T4 was 4.573 times higher than with patients with T1-T2(P<0.001). Vascular tumor thrombus was an independent risk factor affecting the prognosis of patients, and the risk of death was 3.374 times higher than that of patients with negative expression(P<0.001)4 Correlation analysis among the expression of Vav1, MMP-2 and TIMP-1 in gastric cancer showed that there was a positive correlation between Vav1 and MMP-2(P<0.001, r=0.426), while Vav1 were not significantly related to the relationship between Vav1 and TIMP-1(P=0.587, r=0.046) also and MMP-2 and TIMP-1(P=0.220, r=0.104).Conclusions:1 The expression of Vav1 in gastric cancer are higher than the normal tissue. The biger size,lower the degree of tumor differentiation degree, deeper tumor infiltrating,has lymph node metastasis and latter TNM stage, the expression of Vav1 is stronger, suggesting that Vav1 is involved in the invasion and metastasis of gastric cancer.2 Prognostic analysis showed that the prognosis of patients with Vav1 positive expression is lower than that of negative expression, which can be used as an independent risk factor for the prognosis of patients with gastric cancer.3 The expression of MMP-2 in gastric cancer are higher than the normal tissue. The biger size, deeper tumor infiltrating,has lymph node metastasis and latter TNM stage, the expression of MMP-2 is stronger, suggesting that MMP-2 is involved in the invasion and metastasis of gastric cancer.4 Prognostic analysis showed that the prognosis of patients with MMP-2 positive expression is lower than that of negative expression, which can be used as an independent risk factor for the prognosis of patients with gastric cancer.5 The expression of TIMP-1 in gastric cancer are lower than the normal tissue.6 Prognostic analysis showed that the prognosis of patients with TIMP-1 negative expression is lower than that of positive expression. TIMP-1 negative expression can be used as an independent risk factor for the prognosis of gastric cancer patients.7 Correlation analysis showed that there is a positive correlation between Vav1 and MMP-2, while Vav1 are not significantly related to the relationship between Vav1 and TIMP-1 also and MMP-2 and TIMP-1. It is suggested that Vav1 as a tumor related factor, which promotes the development of gastric cancer, and plays an important role in the invasion, metastasis and prognosis of gastric cancer.
