Objective:Hcy which is a sulfur-containing amino acid in the body is generated by methionine demethylation metabolism,Hcy is the product of the methionine metabolism.methylenetetrahydrofolate reductase(MTHFR)is a key enzyme in the metabolism of Hcy.MTHFRC677 T mutation can lead to decrease tetramethylene oxygen folic acid reductase activity,reduce the heat resistance,then affect the body methionine metabolism,lead to Hcy in vivo accumulation,The levels of Hcy in serum were significantly increased,causing high homocysteine hyperlipidemia,To make the body produce a series of related pathophysiological changes.Therefore are great value to explore the relationship between MTHFR gene polymorphism and homocysteine levels in serum and essential hypertension,to clarify the mechanism of Hcy levels and the prevention of Hcy related diseases.In this study,we selected the hypertension patients in our hospital and the healthy physical examination population as the research object,to investigate the changes of serum Hcy levels in patients with H-type hypertension,MTHFR C677 T gene mutation and its role in the occurrence of H hypertension.The aim of this study was to explore the characteristics of MTHFRC677 T mutation in H-type hypertension patients in the region,and to provide basic medical information for the prevention and treatment of patients with H-type hypertension.And to provide accumulation of basic research data for the prevention and treatment of H-type hypertension in our country.Methods:We selected the essential hypertension patients in our hospital From January 2015 to December 2015,through the examination of information,select the primary hypertension patients into the experimental group.According to the determination of the serum Hcy level,The serum Hcy concentration >10 ?mol /L as H-type hypertension group(H group)include 144 cases,with 78 male and 66 female,aged from 26 to 74;Serum Hcy level <10 ?mol /L as the simple hypertension group(simple group),62 cases in simple group,with 33 male and 29 female,aged from 27 to 73;Healthy control group include 110 cases,with 70 male and 40 female,aged from 28 to 69.According to the Chinese guilder for the hypertension prevention and control guidelines in 2010 revised the diagnostic criteria of this text,According to the level of blood pressure,the H type hypertension patients is divided into 1,2,3 grade hypertension group:1 stage hypertension group include 32 patients,and 2 stage hypertension group include 52 patients,and 3 stage hypertension group include 60 patients.There was no significant difference(P > 0.05)between the age and the course of the disease in the groups.Above each group are target group for research,Determination of homocysteine in serum by the method of Sadenosine homocysteine based on small molecule capture technology Circulating enzymatic method was used to measure the serum homocysteine level of each group.Polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)method was used to detect the MTHFRC677 T gene polymorphisms and the gene typing was performed in the subjects.We analyzed the differences of serum Hcy levels between the three groups,and the level of 1,2,3 in Hcy level of H-type hypertension group,and the difference between them.In this study,we detected of MTHFRC677 T genotype frequency and allele mutation distribution,and explored the role of this mutation and genotype variation in Hcy level and the incidence of essential hypertension.Results:1 The levels of homocysteine measured in 1,2,3 stage in the H-type hypertension were:1 stage 21.43?mol/L,2 stage 23.35?mol/L,3 stage 20.35?mol/L;In simple group and Healthy control group,the levels of homocysteine were 8.60?mol/L and 8.39?mol/L.There was no significant difference in H-type hypertension at all levels.But there were significant differences in Hcy levels between the H-type hypertension group and the simple group or the control group.At different ages,the level of serum Hcy in the patients with H-type hypertension was higher than that in the female.The Hcy level in Male was(22.59+1.34)?mol/L and in female was(20.95±1.29)?mol/L.The level of Hcy increased with the increase of the age of the patient.The difference was significant between each group.2 In H-type hypertension and simple group,the frequencies of MTHFR C677 T genotype were C 45.9% vs 67%,T 54.1% vs 33%,CC 22.2% vs 45.2%,CT 47.2% vs 43.5%,TT 30.6% vs 11.3%.The frequency in the control group of CC?CT?TT?C and T was 49.1%?39.1%?11.8%?68.6% and 31.4%.In the above three groups,the frequency of T allele in patients with H-type hypertension was significantly higher than that in the other two groups,the distribution frequencies of CT and TT mutant phenotype were higher than those of the simple hypertension group and the normal control group,and the result was statistically significant(P < 0.05).3 The Hcy level of TT genotype subjects was higher than that of the CT genotype and CC genotype.The carrying rate of TT genotype was 89% in the high homocysteine group,compared to the normal homocysteine group(29.3%)and Levels of Hcy ranged from 10 to 15(35.9%),The differences between the above three groups were statistically significant(P < 0.05).Conclusions:1 There was no significant difference between the levels of essential hypertension and Hcy,but were higher than those in the control group,which suggested that the relationship between homocysteine and hypertension were more closely related.The level of Hcy is significantly correlated with age,and gender,male is higher than female,with age,Hcy levels increased continuously.2 The carrying rate of T allele was significantly higher in H-type hypertension group than the other two groups,which proved that MTHFR C677 T TT gene mutation may be an important genetic factor in the pathogen of H-type hypertension.3 Methylenetetrahydrofolate reductase C677 T gene polymorphism was closely related to serum Hcy level.The changes of TT and T alleles in MTHFR were positively correlated with the elevation of Hcy level in blood,MTHFR C677 T gene polymorphism and serum Hcy levels were all independent risk factors of H-type hypertension.
