| Objective: Among tumors,gastric cancer has the most highest incidence in China.Although in recent years the use of a variety of comprehensive treatment,such as surgery,chemotherapy,radiation therapy and molecular targeted therapy,allows treatment of gastric cancer to make great progress,the five-year survival rate of patients with gastric cancer is still less than 40%.The clinical treatment of traditional anti-tumor drugs become insensitive,which seriously influences the patient’s survival.Therefore,the research on tumor cells against chemotherapy is becoming extremely urgent.Studies have shown that the amount of GRP78,Caspase-12 in a variety of tumor tissues is closely related to the incidence,development and anti chemotherapy treatment of the gastric cancer and they have showed high expression status for it.This study was targeted at clinical entity tissue,in the detection of ERS response associated protein GRP78,Caspase-12 expression in gastric cancer and precancerous lesions,and explored endoplasmic reticulum stress response which occurs in cancer and them,the role and relationship development in order for the clinical treatment of gastric cancer chemotherapy in order to provide a valid target.Methods: 95 cases were studied,and their clinical data were collected,including 30 cases of gastric cancer tissues wax block,25 cases of stomach and severe dysplasia wax block,25 cases of intestinal metaplasia wax block,15 cases of superficial gastritis wax block tissue microarrays.Immuno-histochemistry was used to measure levels of the expressions of GRP78,Caspase-12.Results: GRP78 expression and Caspase-12 expression can be seen in gastric tumor,gastric severe dysplasia,intestinal metaplasia.GRP78 localized in the cytoplasm and Caspase-12 localized in the nucleus,colored pale yellow to dark brown.The statistical results show:(1)The positive expression rates of GRP78 in chronic superficial gastritis tissues,intestinal metaplasia,dysplasia and gastric tumor were 13.3%,48.0%,80.0% and 90.0% respectively.Comparison of GRP78 expression in superficial gastritis and intestinal metaplasia,intestinal metaplasia and atypical hyperplasia was significantly(P = 0.020,0.008),the difference in dysplasia and gastric carcinoma was not statistically significant(P = 0.445).In well-differentiated(G1)and poorly differentiated(G2 / G3)in GRP78 expression rate of gastric carcinoma were 57.1%(4/7)and 100%(23/23),(P = 0.006).In phase I + II and III + IV gastric cancer tissues GRP78 positive rates were 50.0%(3/6)and 100.0%(24/24),(P = 0.007).Rates with lymph node metastasis and without lymph node GRP78 gastric carcinoma metastasis-positive were 100%(20/20)and 80.0%(8/10),(P = 0.023)(2)The positive expression rates of caspase-12 in superficial gastritis,intestinal metaplasia,dysplasia,gastric carcinoma were 0%,44.0%,52.0% and 30.0%respectively.Caspase-12 expression difference in superficial gastritis and intestinal metaplasia was statistically significant(P = 0.009),the difference in intestinal metaplasia and dysplasia,dysplasia and gastric carcinoma was not statistically significance(P = 0.777,0.083).Caspase-12 expression rates of well differentiated(G1)gastric cancer and poorly differentiated(G2 / G3)gastric cancer were 71.4%(6/7)and 13.0%(3/23),(P = 0.001).Caspase-12 positive rate with lymph node metastasis and without lymph node metastasis in gastric carcinoma were 15.0%(3/20)and 60.0%(6/10),(P = 0.022)(3)In the intestinal metaplasia,GRP78 and Caspas-12 showed a positive correlation(P = 0.000,r = 0.935),the dysplasia and gastric carcinoma had no significant correlation.Conclusions:(1)Both in gastric carcinoma and precancerous lesions,there were abnormal expressions of GRP78,Caspase-12,which was related with the clinical pathological features of gastric carcinoma.(2)Endoplasmic reticulum stress continued during the pathogenesis of gastric cancer,in which GRP78,Caspase-12 and endoplasmic reticulum stress response may play an important role.It will benefit for the detection and treatment of gastric cancer if detected early stage precancerous changes in the correlation index and intervention. |
