| Objective: To investigate the expression and its clinical significance of SDF-1 and VEGF in synovial sarcoma.Method: A retrospective analysis of 54 synovial sarcoma patients treated by operation(with complete follow-up data,pathology)in our hospital from Jan 2004 to Jan 2011.Result:1.SDF-1 and VEGF of high level expression in synovial sarcoma were 24 cases and 22 cases.Moderate positive expression were 19 cases and 20 cases.Weak positive and negative expression were 11 cases and 12 cases.SDF-1 of medium/high expression rate in synovial sarcoma was 79.6%.VEGF medium /high expression rate was 77.8%.2.The different expression of SDF-1 in 54 cases of synovial sarcoma tissues and histopathology grading(χ2=15.35,P=0.004)had relevance: clinical staging(the expression level of SDF-1 III,IV tumor tissue was higher than that of I,II,χ2=24.81,P < 0.001);metastasis(χ2=9.49,P=0.009);but had no significant correlation with gender,age,tumor size(P > 0.05).VEGF expression had relevance with histopathology grading(χ2=9.93,P=0.042);clinical staging(the expression level of SDF-1 III,IV tumor tissue was higher than that of I,II,χ2=11.65,P=0.003);metastasis(χ2=7.17,P=0.028);but had no significant correlation with gender,age,tumor size(P > 0.05).3.The expression levels of SDF-1 and VEGF of patients with synovial sarcoma tissue had significant positive correlation,the correlation coefficient was 0.612 and has statistical significance(P < 0.05).Immunofluorescence results show that SDF-1(red)and VEGF(green)in the cytoplasm and cell membrane,showed high intensity fluorescence,confocal results show they exist at a synovial sarcoma.4.Univariate regression analysis showed: age,sex,tumor size and histological grading and patient survival time(OS)had no significant correlation(P> 0.05),and tumor metastasis[P =0.09,HR=2.452(1.247~4.820)],clinical staging [P < 0.01,HR=2.386(1.673~3.401)],and SDF-1 [P < 0.01,HR=4.685(2.557-8.585)] and the expression of VEGF[P = 0.01,HR=2.018(1.316~3.094)] had influence on survival time,which has statistical significance(P < 0.05).5.The results of multivariate regression showed that gender,age,tumor size,histopathologic grading,the expression of VEGF had no significant effect on survival time of patients.Synovial sarcoma metastasis[P =0.017,HR=0.185(0.046-0.737)],clinical stage[P =0.004,HR=3.680(1.519-8.914)] and SDF-1 expression [P =0.026,HR=2.640(1.124-6.200)] had significant effect on total survival period,which was statistically significant.6.At the expiration of five years of follow-up,5 year survival rate of 54 cases of synovial sarcoma patients was 19.76%.SDF-1 of high expression in synovial sarcoma was 24 cases(44.4%),moderate expression was of 19 cases(35.2%),and low expression and negative expression were of 11 patients(20.4%),the expression of SDF-1 and prognosis have obvious correlation.The higher the expression is,the shorter the survival time will be,which has statistical significance(Plogrank < 0.01).And VEGF in synovial sarcoma of high expression was of 22 cases(40.7%),moderate expression was of 20 cases(37%),and low expression and negative expression was of 12 cases(22.2%);the survival time of high expression patients was significantly lower than that of the low / no expression patients,which has statistical significance(Plogrank < 0.01).Conclusion:1.Positive expression rate of SDF-1 and VEGF in synovial sarcoma is high.2.There was no significant correlation between the expression of SDF-1 in synovial sarcoma tissue and age,sex,tumor size of patients.But there was correlation between it and histological grading,metastasis and clinical staging,and expression of SDF-1 in synovial sarcoma tissue showed higher expression;there was no significant correlation between expression of VEGF in synovial sarcoma tissue and age,sex,tumor size of patients.But there was correlation between it and histological grading,metastasis and clinical staging.3.The expression of SDF-1 and VEGF was positively correlated.4.The survival time of patients with synovial sarcoma showed significant differences with different expression of SDF-1 and VEGF,and the higher the expression level of SDF-1 and VEGF is,the shorter the survival time of the patients will be. |
