Research On The Application Rules Of Xiao Chaihu Decoction In The Three Volumes Of Classified Medical Records Of Distinguished Physicians

Objective:This thesis aims to research the application rules of prescriptions in Treatise on Cold-Induced and Miscellaneous by physicians of the prosperity Diseases.The conclusion of the tongue characteristics,pulse manifestation,appear syndrome,square frequency,etc,that present in the application of Xiao Chaihu Decoction in the three volumes of Classified medical record Records of Distinguished Physicians draws forth the application rules and the spectrum of diseases which provides ideas and reference to modern clinical use.Methods:First,medical records that successfully applied the method and prescriptions of Treatise on Cold-Induced and Miscellaneous in the three volumes of Classified medical record Records of Distinguished Physicians are selected in reservation.Second,After analyzing and counting of the types and appearance frequency of diseases presented in the successful medical records,Xiao Chaihu Decoction is picked out as the research target,preliminary study of its application rule and frequency is then set out to deal with.This thesis compares the differences between the clinic symptoms of Xiao Chaihu Decoction in some specific medical records and the description of the symptoms in the clauses of Treatise on Cold-Induced and Miscellaneous.The thesis mainly studies the tongue characteristics,pulse,modification of the medicine,combined formula and the Similarities and differences of symptoms that present in the selected medical records.Results:1 Clinic Application Guidance of Xiao Chaihu Decoction Tongue characteristics: mainly present reddened tongue or with yellow fur.Pulse manifestations: the 5 manifestations of pulse that may appear in medical records of Xiao Chaihu Decoction Symptoms are listed in the principle of frequency as wiry pulse,rapid pulse,deep pulse,surging pulse and slippery pulse.With the developing of the spectrum of the diseases and extension of the application range of the decoction,rapid pulse,surging pulse and slippery pulse are considered as more common pulse manifestations.Symptoms:The disease may present with set of manifestations as fever,alternate attack of chill and fever,rib pain,headache,aversion to cold,vomiting,delirium,thirst,sweating,bitter taste in the mouth,difficulty in micturition,urine red,late afternoon tidal fever,deafness.secondary symptoms of Xiao Chaihu Decoction Syndromes may appear profuse phlegm,in low spirit with no appetite,swelling of ear,thirsty,stomachache,diarrhea,cough,constipation,lockjaw,pain and fullness in lower abdomen,fullness and stuffiness in the chest,cloudy urine,mastodynia,pain and swelling in penile,pain in low abdomen,irritability,bluish complexion,dizziness,coldness in hands and feet.2 Application of Modified pharmaceutical drugs: 22 types of modified pharmaceutical drugs are found in the 207 medical records of Xiao Chaihu syndromes.There are 82 drugs of them and appear 230 times in those medical records.The 22 types can be divided as heat-clearing herbs,exterior-releasing herbs and herbs to regulate the flow of qi.The common drugs of heat-clearing herbs are Rhizoma Anemarrhenae,Coptis chinensis Franch,Rehmannia glutinosa Libosch,cortex moutan,Gardenia jasminoides Ellis,gypsum;Akebia quinata(Houtt.)Decne and Alisma plantago-aquatica Linn are used to increase urine excretion.Common kinds of exterior-releasing herbs include Ramulus Cinnamomi and mediciines used to promote blood circulation such as Semen Persicae,Szechwan Lovage Rhizome,Radix Paeoniae Rubra,Flos Carthami;medicines that provide medicinal substances for resolving phlegm such as Radix Platycodonis,Fructus Trichosanthis.Medicines for regulation of the flow of qi include Fructus Aurantii Immaturus,Cortex Magnoliae Offcinalis,Semen Arecae,Nutgrass Galingale Rhizome;medicines to restore qi such as Radix Ophiopogonis,Carapax Trionycis;antimalarial drugs as Radix Dichroae;drugs to enrich blood as White Peony Root and Chinese Angelica;drugs to strain off dampness as Poria Cocos.3 Study of Combined Formula: the Main function of Xiao Chiaihu Decoction is to relieve symptoms of Shaoyang Meridian and a modified formula provides better effect to deal with the diseases and expand the range of disease entity.Modified Formula of Xiao Chaihu Decoction: 4 medical records modified with Siwu Decoction to enforce the effect of enrich blood;2medical records modified with Luhui Pills to enforce the effect of clearing liver heat and purging fire during the treatment for swelling and pain in external genitalia;1 medical record modified with Daochi Powder to enforce the effect of clearing heat and increasing secretion of urine;1 medical record modified with Gualou Powder to enforce the effect of resolving phlegm and disintegrating masses;1medical record modified with Xiaoxianxiong Decoction to treat warm pathogen disease;1 medical record modified with Dayuan Decoction to treat pestilence;1 medical record modified with Danggui Huangqin Decoction to enforce the effect of clearing heat and regulating the blood;1 medical record modified with Banxia Xiexin Decoction and Xiaoxianxiong Decoction to treat the fever;1 medical record modified with Xiaojianzhong Decoction and Miaoxiang Jingong Powder to enforce the effect of strengthening the middle-energizer;1 medical record modified with Qinghun Powder and Shenghua Decoction to enforce the effect of eliminating pathogenic factors to restore the healthy qi.Those modified formulas have a better effect in clinical treatment which expense the application range of Xiao Chaihu Decoction.4 Spectrum of Disease: in the 207 application medical records of Xiao Chaihu Decoction,main indications are surgical diseases,external coontraction,gynaecopathia and spleen and stomach disorder.In the three volumes of Classified medical record Records of Distinguished Physicians,Xiao Chaihu Decoction is found the treat medical problems such as cold-induced medical records,diarrhoea,dysentery,scrofula,hypochondnrium,acute mastitis,malaria,rib pain,latent summer-heat,swolllen knee,chancre,spring warmth,disease caused by summer-heat and dampness,catch cold,dysmenorrhea,invasion of the blood chamber by heat,uterine bleeding,cough during gestation period,afterbirth disease,acute infantile convulsion,hernia,hemorrhoids,external genitalia disease,uvular abscess,tuberculosis,multiple abscess,lepra,syphilis,macula and pyogenic skin infection,bleeding of live fire,stomachache,wasting-thirst disorder,hematemesis,hematochezia,stagnation syndrome,impediment syndrome,phlegm syndrome.Conclusion : Prescriptions of Treatise on Cold-Induced and Miscellaneous Diseases have been widely applied in many diseases in posterity and the range of disease entity has been extensively developed.In addition to the differentiation of syndromes in accordance with the theory of the six meridians of the decoctions,differentiation of Zang-fu syndrome and syndromes according to the theory of meridians are also considered and applied in diagnosis.To get better use of the decoctions,modified formula of Xiao Chaihu Decoction are widely applied to treat surgical diseases,external coontraction,gynaecopathia,spleen and stomach disorder diseases.(1)From Ming and Qing Dynasties to Minguo Period,spectrum of disease reached a research peak by physicians of classic herbal formula.Most of the medical records of the second volume of Classified medical records.Records of Distinguished Physicians are recorded in this period.The application range of The classic herbal formulas of Treatise on Cold-Induced and Miscellaneous Diseases spreads from treatment of narrowed sense of cold-induced disease to warmth disease,internal diseases,surgical diseases,pediatric disease and gynaecopathia.(2)Xiao Chaihu Decoction is often used with its regular formula,the modification of the dosage and proportion of the medicines according to the pathogenesis and modified formula create new effect which greatly expanded its clinical application range.(3)To provide medication guide,in the Tang Period,under the principle guidelines of Guizhi Decoction,Mahuang Decoction and Qinglong Decoction,Sun Simiao classified diseases by combining prescriptions with clinical manifestations instead of the former classification of Zhang Zhongjing who defined the diseases by the differenciation of syndromes in accordance with the theory of the six meridians.His new classification actually narrowed the application range of the prescriptions.Then Song Zuhong published his medical book Nan YANG Treatise in which he had combined cold-induced prescriptions with the differenciation of syndromes according with the theory of meridians.He first put forward the medication guide according to the meridians syndromes and emphasized the importance of gathering the medical history by asking.His method made the prescriptions more suitable in clinical application though it wasn’t widely used in surgical diseases.For example,medical records of Xu Shuwei in the second volume of Classified medical records.Records of Distinguished Physicians are mainly basic application of the original clause and the theory of Treatise on Cold-Induced and Miscellaneous Diseases with less modification of the prescriptions.In the period of Jin and Yuan Dynasty,more schools of traditional Chinese medical science emerged with more extensive explanation and application of the prescriptions which had a deep influence to the posterity.In the medical record of how to cure a patient with edema syndromes,Mahuang Fuzi Xixin Decoction of Zhang Zhong Jing and Wupin Decoction are combined with Zu Danxi’s theory who attributed the etiology of edema in feet to water.New effect was created by the new combination.In the period of Ming and Qing Dynasty,more medical schools emerged,in which,an important school called school of seasonal febrile disease came into formation.This school applied a large number of the original prescriptions of Treatise on Cold-Induced and Miscellaneous Diseases under the guideline of modified theory.Physicians tested and verified the theory effectiveness of treating the patients by dignosing the differenciation of syndromes according with the theory of meridians.They applied the prescriptions in surgical diseases under the guideline of the moving mode of the meridians and the symptoms that the prescriptions mainly applied.Before the Song Dynasty,the common application of the prescriptions was to use the prescriptions according to the symptoms that presented.After the Song Dynasty Period,physicians began to research the mechanism of action of Xiao Chaihu Decoction and tried to explain mechanism from the poiint of the physiological and pathological characteristics of the Zang-fu organs and the main symdromes of the meridians.On this basis,the application of Xiao Chaihu Decoction got extensive development and the guideline of the application of Xiao Chaiu Decoction finally consists several theories such as theory that applies the prescription according to the original clause,theory that applies the prescription by the mutual generating and restraining among the five phases of Zang-fu organs,sequential meridian transmission theory,etc.Clinical application got great development on the basis of the diversification of ideas.We are enlightened that during the research of the application and law of the prescriptions,we should try to process our research from different angles with new methods,not limited to the original clauses and the theory of the six meridians.
