The development of primary health care is a basic way to solve the medical problems and key to provide equal medical service, which is an important aspect of the construction of the socialist harmonious society. To develop primary health care, it is necessary to learn lessons from other countries. Medical problems in America is worldwide concerns and the innovation mode "Patient Centered Medical Home" has played a positive role in solving medical problems. Compared with countries of developed primary health care, there is no strictly "gatekeeper" in America. "Medical Home" mode is to solve practical problems in recent years. "Medical Home" is of value for the development of China's primary health care.This paper studies the construction of primary health care mode in urban and rural areas based on the experience of "Patient Centered Medical Home" and the current situation of our country. The full paper is divided into five parts. The first part is the introduction of research background, significance, research status, main research content and method, innovations and deficiencies. The second part is defining primary health care and introducing related theory. The third part is the analysis of the "Medical Home" mode from the aspects of background, connotation, operation process and support system. Its achievements are also introduced. The development of China's primary health care in recent years is summarized in fourth part.The problems in primary health care are also introduced. The fifth part is conclusion, based on the existing problems and "Medical Home" mode experience, the construction of primary health care mode is introduced in the aspects of principles, service content, service organization, the source of funds and payment method. To build the mode, the key points are the participation of residents, legislation, training doctors, information technology development, regulation and risk prevention.
