Objective: To evaluate the application of 3D-ASL and DTI in the internal carotid artery system.stenosis disease.Materials and Methods: 22 patients with unilateral ICA severe stenosis or occlusion in our department of neurology from March 2015 to October 2015 were collected.All patients were performed 3D-ASL and DTI in 3.0T MRI.The patients were divided into severe stenosis group and occlusion group according to the NASCET criteria.ROI was sited in the bilateral anterior watershed WM,middle cerebral artery WM,posterior watershed WM of 3 different slices: centrum semiovale,lateral ventricle,fundus.CBF and FA were measured;CBF and FA in 3 slices were averaged,compute the CBF ratio,the FA ratio simultaneously.Data were analysed by SPSS 17.0,including Independent/Paired-Samples T-Test,ANOVA and rank correlation.Results: 1)In severe stenosis group,there were statistical significances on CBF between ipsilateral and contralateral in 3 different positions[(34.57±12.33)/(38.97±10.46),(36.10±10.16)/(40.80±7.82),(31.71±11.01)/(39.50±10.45),p<0.05];In occlusion group,there were statistical significances on CBF between ipsilateral and contralateral in 3 different positions[(26.38±10.04)/(34.61±8.17),(28.78±10.14)/(36.08±7.79),(22.86±8.43)/(32.21±8.59),p<0.05];No statistical significances on r CBF was found among the 3 different positions in the severe stenosis group[(0.87±0.15),(0.88±0.11),(0.79±0.13),p>0.05];No statistical significances on r CBF was found among the 3 different positions in the occlusion group[(0.77±0.23),(0.79±0.19),(0.71±0.13),p>0.05];No statistical significances on r CBF of same position was found between severe stenosis group and occlusion group[(0.87±0.15)/(0.77±0.23),(0.88±0.11)/(0.79±0.19),(0.79±0.13)/(0.71±0.13),p>0.05];There were statistically no correlation between the value of r CBF and the degree of artery stenosis(r=0.238、0.216、0.353,p>0.05).2)There were statistical significances on FA between ipsilateral and contralateral in 3different positions of occlusion group[(0.25±0.05)/(0.30±0.03),(0.25±0.09)/(0.36±0.04),(0.28±0.05)/(0.33±0.03),p<0.05];There were statistical significances on FA between ipsilateral and contralateral only in anterior watershed of severe group[(0.26±0.04)/(0.30±0.06),p<0.05];No statistical significances on r FA was found among the 3 different positions in the severe stenosis group[(0.87±0.11),(0.96±0.16),(0.91±0.22),p>0.05];No statistical significances on r FA was found among the 3 different positions in the occlusion group[(0.83±0.18),(0.73±0.29),(0.84±0.18),p>0.05];There were statistical significances on r FA between severe stenosis group and occlusion group only in middle cerebral artery region[(0.96±0.16)/(0.73±0.29),p<0.05];There were statistically positive correlation between the value of r FA and the degree of artery system stenosis only in middle cerebral artery region(r=0.504,p<0.05).3)There were statistically no correlation between the value of r CBF and the value of r FA in the same position(r=0.159、0.212、0.353,p>0.05).Conclusion: 3D-ASL and DTI can detect the variety of cerebral perfusion and the microstructure of white matter fiber bundles,they can evaluate quantificationally the changs of cerebral perfusion and the degree of WM fiber bundles damage,they can offer great guidance for the diagnosis and treatment of the internal carotid artery system.stenosis disease.
