The Effect Of YiShenJiangZhuo Granule On Chronic Kidney Disease,Serum FGF23 And Serum IPTH Of Spleen And Kidney Qi Deficiency And Dampness Syndrome

Objectives:To observe the clinical curative effect of YiShenJiangZhuo granule on patients with chronic kidney disease of spleen and kidney qi deficiency,and detect the influence of YiShenJiangZhuo granule on serum FGF23 and serum iPTH,then analyse the correlation between the observation indexes,to discuss the influence of the disorders of calcium phosphate metabolism in patients with chronic kidney disease (CKD) by YiShenJiangZhuo granule,and provide objective evidence for YiShenJiangZhuo granule treating on the disorders of calcium phosphate metabolism and secondary hyperparathyroidism (SHPT) in patients with chronic kidney disease (CKD)Methods:96 cases of patients with CKD3 and TCM syndrome differentiation of Spleen and Kidney Qi Deficiency and Dampness Syndrome were enrolled as the object of study,and randomly allocated into three groups,Each group had 30 cases. Patients i-n A group were treated with pure western medicine,patients in B group were treated with NDQ and western pure medicine,patients in C group were treated with YiShenJia-ngZhuo granule and pure western medicine,and observe two months of continuous treatment. FGF23, iPTH, BUN, Scr, Ca,P levels were determined, then GFR was calcula-ted based on MDRD equation. To observe the level of total curative effect, the level of TCM syndrome effect,the level of the TCM syndrome integral?Results:1.The level of Scr?eGFR and P after treatment was very significant diff-erence compared with the level before treatment(P<0.01). Among the groups,it had si-gnificant difference between A and B,C group(P<0.05). It had insignificant difference betwe-en B and C group(P>0.05).2. Compared with normal group, the level of FGF23 had significant difference between A and B,C group(P<0.01).The level of serum FGF 23 after treatment was very significant difference compared with the level before treat-ment(P<0.01). Among the groups,it had significant difference between A and B,C grou.-p(P<0.05). It had insignificant difference between B and C group(P>0.05).3. The leve-1 of serum PTH after treatment was very significant difference compared with the lev-el before treatment(P<0.01).And it had insignificant difference among the groups (P>0. 05).4. The level of PTH after treatment was not significant difference compared with t he level before treatment(P>0.05).5. correlation analysis:The level of serum FGF23 sho wed positive correlation with Scr.P (P<0.05) correlation with GFR (P<0.01),no corr elateon with Ca and iPTH (P>0.05).6.The level of total curative effect showed signif ycant difference between A group and B,C group, insignificant difference between B a nd C group(P<0.05).The level of TCM syndrome effect showed significant difference between each two groups.The level of the TCM syndrome integral was very significa nt difference compared with the level before treatment(P<0.01). Among the groups,it h ad significant difference between A and B,C group(P<0.01). It had significant differenc e between B and C group(P<0.05).Conclusions:1.The level of serum FGF23 of Spleen and Kidney Qi Deficiency and D ampness Syndrome is higher than healthy people.2. The level of serum FGF23 showed correlation with renal function, the serum FGF-23 level is higher as the renal functio n be worse. So it's important to inhibit FGF23 expression in CKD patients.3.YiShenJ iangZhuo granule and NDQ can improve the level of serum FGF23 and it is more eff ective than pure western medicine, Consider Yishenjiangzhuo or Niaoduqing may impr-ove the renal function, and to improve the metabolism of calcium and phosphorus effe-ct.4. YiShenJiangZhuo granule and NDQ can improve the total curative effect and it i s more effective than pure western medicine.And YiShenJiangZhuo granule shows mor e effective than NDQ in improving TCM syndrome integral and TCM syndrome effect. YiShenJiangZhuo granule is effective on treating CKD and deserves more promote.
