ObjectiveTo analysis the Correlation of clinical symptoms of lumbar disc herniation and MRI imageing and to study the curative mechanism of EA at Jiaji points against lumbar disc degeneration though observing effection on expression of AQP1、3 in rabbit degenerated disc Methods1. 90 cases meeting the criteria of lumbar disc degeneration,use Oswestry disability Index(Oswestry disability Index, ODI) to rate of patients with clinical symptoms, imaging data collection, using the statistical analysis of the correlation of clinical symptoms and imaging performance.According to the index of the ODI score divided the patients into three groups, mild group score < 15 points; Moderate group score 15 ~ 30 points;Severe group score > 30 points; 51 points, total score.According to the imaging performance, select outstanding size,pathological type, degree of nerve root compression as three indicators observed. For a case with multiple lumbar disc, take the most severe and oneof the most consistent with the symptoms of intervertebral disc were analyzed.2. Forty-four New Zealand white rabbits were randomly divided into 5groups :normal group, sham operation group, model group, EA group,EA+blockers group, 4 in normal group, the remaining 10 in each group. We establish the rabbit model of lumbar disc degeneration refer to Kroeber M,Gao Yu and guo-fu huang ’ s building method by stressing berween the vertebral body. The model group accept product ion of LIDD model without any treatment. The sham operation group accept the same processing but without Intervertebral pressure,also without any treatment.EA group accept EA treatment since the first day after model made. The EA+ blockers group accept the same processing as the EA group and be injected AQPs inhibitors at the first day.Take MRI test and test dynamic expression of AQP1, 3 at the 1st day and 28 th day.Results Clinical research1.According to index of the ODI, mild group of 25 cases, moderate group of42 cases, 23 cases of severe group.Grouped by herniation type, the central type of 31 cases, including 14 cases of grade mild,11 cases of moderate, 6cases of severe; Side type, a total of 59 patients, mild scoring 11 cases, 31 cases of moderate, 17 cases of severe. According to the relation with nerve root,24 cases without pressure, compression group 45 cases, adhesion group21 cases.2. Statistical analysis results show that the clinical symptoms in patients with correlation with the degree of nerve root compression,highlighting the more close to the nerve root position of nucleus pulposus, the heavier clinical symptoms, and the more severe patients; the clinical symptoms in patients with correlation with the herniation type, type side herniation with heavier symptoms in patients; Patients with clinical symptoms and protrusions size has no obvious correlation.Experiment research Using western blot test AQPs expression of intervertebral disc tissue, on day 28, compared with normal group and sham operation group, the two kinds of protein expression levels of model group, EA group and EA+blockers group were significantly decreased, indicate that the AQP1, AQP3 expression level decreased obviously in degenerated lumbar disc tissue.The EA group compared with model group, EA+ blockers group, no difference between the two kinds of protein expression on day 28. After EA treatment on day 56, compared to time points of day 28, two kinds of protein expression of model group has no obvious change, protein expression levels of EA+blockers group are down a bit, two kinds of protein expression levels of EA group were obviously higher, indicating that EA at jiaji points can be effectively raised AQP1, AQP3 expression.Conclusion1.lumbar intervertebral disc protrusion to the side type,with heavier symptoms,patients with central type herniation symptom is lighter. Patients with clinical symptoms mainly related to the degree of nerve root compression and herniation type, with protrusions size has no obvious correlation. Nerve root compression, the more obvious, the heavier patients clinical symptoms.2. The AQP1, AQP3 expression level decreased obviously in degenerated lumbar disc tissue, EA at jiaji points can be effectively raised AQP1, AQP3 expression. |
