| Objective Traumatic brain injury is a common neurosurgical disease. The incidence of traumatic brain injury accounts for about 1/5 of the body parts of the trauma, and the disability rate is the first. Traumatic brain injury is divided into light, medium and severe by injury severity. Severe Traumatic brain injury have the symptoms of extensive brain contusion,extensive skull fracture, brainstem injury, diffuse axonal injury or intracranial hematoma and others. The mortality rate of Severe traumatic brain injury is very high. It is a severe illness, its condition changes quickly, has sequelae, high morbidity, seriously affects the lives and quality of patients. So Severe traumatic brain injury has been the focus in neurosurgery research at present. Recently, HBO has been used to treat brain injury of different severity and different stages. HBO can rapidly improve and correct brain tissue hypoxia, reduce brain edema and intracranial pressure, improve nerve function, reduce complications and improve the prognosis and the quality of life. But there is still no clear report about the timing of the HBO therapy can achieve the best effect. In this study, we mainly researched and analyzed the treatment effect of HBO of severe traumatic brain injury. We compared the clinical efficacy of HBO and the conventional treatment. And analyzed the many factors that influence the effect of HBO using statistical analysis. At last providing useful experience for clinical HBO treatment of severe traumatic brain injury.Methods This study retrospectively analyzed 72 cases of patients of severe traumatic brain injury, HBO treatment group 36 cases, conventional treatment group 36 cases, and compared the efficacy of two groups. Analyzed the common factors affecting the efficacy of HBO: gender, age, cause of injury, admission GCS score, tracheotomy or not, the first time of HBO after injury and HBO therapy treatment course altogether seven factors. Grouped according to their main factors for comparison effect.The factors were analyzed using Logistic Regression.Results 1. Comparative the efficacy of the HBO group and conventional treatment groups: the treatment effects of the HBO group and the conventional treatment group were significantly different(χ2=11.986,p=0.008); The patients of HBO group recovered better than the conventional treatment group(p <0.05), while the vegetative state and mortality were significantly lower than the conventional treatment group(p <0.01). The severe disability rate of HBO group is higher, but not statistically significant(p>0.05) 2. Effects of the HBO influential factors Analysis: The may affect factors: gender, age, reason for traumatic brain injury, admission GCS score, the first time of HBO after brain injury, tracheotomy or not, the course of HBO. The results showed that: the statistically significant factors were whether tracheotomy after traumatic brain injury, admission GCS score and the first hyperbaric oxygen therapy time just admitted. The course of HBO has no statistical significance.Conclusion 1. Hyperbaric oxygen therapy group(HBO group) were significantly different(p <0.01) with the conventional treatment group, hyperbaric oxygen therapy group recovered well, plant survival and mortality rate were reduced. 2. The main factors affecting the efficacy of HBO include: admission GCS score, tracheotomy or not, and the first time of HBO after injury. Admission GCS score higher, the first time HBO earlier, the better the effect. Advising patients to perform HBO therapy in stable condition after one week. And the number of course depends on the patient’s condition and recovery. |
