| Object:This study aims to investigate the correlation of 25-hydroxyvitamin D3 and insulin resistance with papillary thyroid carcinoma. Method:Randomly select 358 patients diagnosised with PTC during 2014.9-2015.9 in the First Affiliated Hospital of Zhengzhou university as observation group.The control group consists of 290 healthy volunteers. Detect the 25-hydoxy vitamin D3、fasting blood glucose、fasting insulin of all the members and explore the relationship between 25-hydroxyvitamin D3、insulin resistance and expression of BRAFV600 E 、lymph node metastasis in PTC. Results:(1) Comparison between PTC and the normal control group:the 25-hydoxy vitamin D3 concentration in PTC(15.8±5.00)(ng/ml)was significantly lower(P=0.000)than the control(19.1±5.66)(ng/ml); the IRI in PTC(1.03±0.220)was significantly higher(P=0.000)than the control(0.965±0.187);(2) Comparison between PTC with and without lymph node metastasis: the 25-hydoxy vitamin D3 concentration in PTC with lymph node metastasis(13.3±4.68)(ng/ml)was significantly lower(P=0.000)than the PTC without lymph node metastasis(16.7±4.82)(ng/ml); the IRI in PTC with lymph node metastasis(1.18±0.191)was significantly higher(P=0.000)than the PTC without lymph node metastasis(0.985±0.207); the age in PTC with lymph node metastasis(40.4±13.0)was significantly lower(P=0.002)than the PTC without lymph node metastasis(45.2±11.1).(3) Comparison between PTC with and without BRAF genetic mutation: the 25-hydoxy vitamin D3 concentration in PTC with BRAF genetic mutation(15.3±4.91)(ng/ml)was significantly lower(P=0.000)than the PTC without BRAF genetic mutation(18.6±4.52)(ng/ml); the IRI in PTC with BRAF genetic mutation(1.08±0.196)was significantly higher(P=0.000)than the PTC without BRAF genetic mutation(0.809±0.199); there was no significant difference between two groups of age.(4) Binary logistic regression analysis showed insulin resistance and female were risk factors of PTC,while 25-hydoxy vitamin D3 was protective factor. Conclusion:25-hydoxy vitamin D3 may be a protective factor for PTC. Improving the insulin sensitivity may help prevent the occurrence and development of PTC. |
